Case Manager

Sunstone Way is seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for engaging with individuals experiencing homelessness, assessing their needs, and developing personalized service plans to help them achieve stability and independence.
Key Responsibilities:
- Conduct comprehensive assessments of clients' needs and develop individualized service plans
- Provide case management services, including referrals, advocacy, and support
- Collaborate with community partners to ensure clients receive comprehensive services
- Monitor client progress and adjust service plans as needed
- Document client interactions and service delivery in a timely and accurate manner
Requirements:
- 1-2 years of experience in case management or a related field
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of housing first and harm reduction models
- Ability to maintain confidentiality and handle sensitive information
Preferred Qualifications:
- Bachelor's degree in social work or a related field
- Experience working with homeless populations
- Knowledge of community resources and services
