Join Our Team as a Case Manager

Service Coordination, Inc. is seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will play a vital role in providing support and advocacy to individuals with Medicaid Home and Community Based services.

Key Responsibilities:
  • Establish positive relationships with individuals and their support systems
  • Develop and implement Person-Centered Plans of Service (POS) to meet individual needs
  • Coordinate and monitor services to ensure quality and compliance with state standards
  • Advocate on behalf of individuals and provide education on self-determination and empowerment
Requirements:
  • Bachelor's or Master's degree in a human service field (e.g., Psychology, Sociology, Social Work, Nursing, Counseling, Human Services Administration)
  • Valid driver's license and insured vehicle
  • Experience working with individuals with disabilities and person-centered planning preferred
