We are looking for an experienced MDS Nurse - RN - Registered Nurse or LPN - Licensed Practical Nurse.
** $33.43 per hour to $41 per hour based on skills and experience. **
MDS Nurse - RN - Registered Nurse or LPN - Licensed Practical Nurse Job Qualifications:
Current licensure in the State of Massachusetts as a Registered Nurse.
Minimum of 1 year of MDS Nurse experience in a long-term care setting.
AANAC certification (RAC-CT) or the ability to obtain AANAC certification within 6 months of hire.
Current BLS certification.
MDS Nurse - RN - Registered Nurse or LPN - Licensed Practical Nurse Summary:
Interact with providers, insurance organizations, coordinating healthcare facilities, families and residents to resolve issues and provide clear communications.
Work to ensure safe transitions through care for new admitting, re-admitting and discharging residents and allow the resident maximum benefit of their Medicare, Medicaid or other insurance benefit.
Monitor Point Click Care “Care Management” areas to ensure accuracy and completeness of resident assessments.
Monitor ABILITY programs dashboards (InfectionWATCH, RiskWATCH & CareWATCH) and maintain policies and for a comprehensive Reimbursement Program.
Case mix clinical condition tracking.
Implement auditing mechanisms.
Oversee and assist with appropriate corrective action of identified deficiencies while reviewing Policies and Procedures.
Cooperate in surveys and inspections.
Ensures 672/802 is updated in PCC on a regular basis within required timeframe.
Review CASPER Reports for accuracy.
Ensures documentation is retained per home policy.
Assist in the retrieval of case mix scores.
