RN Case Management/Social Services Director

Job Summary: Monitors and compiles information on the necessity of the admission of patients to the hospital and on the continued stay of patients by reviewing patients# records. Assures the hospital and medical staff complies with federal and state requirements for quality of patient care and optimum utilization. Coordinates patient care across the continuum to assure quality of care in a fiscally responsible environment. Qualifications/Reporting: Required minimum education: Bachelor of Science in Nursing is preferred or will substitute Associate Degree in Nursing with four years of experience required. Required minimum experience: Two years# to Four years# experience in a hospital setting, depending on education. Experience in quality management or case management activities preferred. Data analysis preferred. Required skills (if applicable): Knowledge of medical terminology and effective oral and written communication abilities. Familiarity with NCT#criteria a plus. Familiarity with coding concepts and coding software desired. Knowledge of age-specific needs and the elements of disease processes and related procedures. Strong broad-based clinical knowledge and understand of pathology/physiology of disease processes. Excellent written and verbal communication skills. Excellent critical thinking skills. Excellent interpersonal skills to build effective partnering relationships with physicians, nurse staff, hospital management staff, and health information staff. Ability to work independently in a time-sensitive environment. Computer literacy and familiarity with the operation of basic office equipment. Assertive personality traits to facilitate ongoing physician communication. Ability to stand and walk for periods of time is required in the performance of job duties. Working knowledge of Medicare reimbursement system and coding structures/national coding guidelines. Required license/certification: RN currently licensed to practice in the State of Texas. Job reports to (title of the position): Chief Financial#Officer Job titles reporting to this position: Case Manager, Utilization Review, Administrative Assistant, and Social Services.
