PROGRAM OVERVIEW:
PersonLink is a supports coordination organization (SCO) serving individuals with intellectual disabilities in the Greater Philadelphia region. Our compassionate Supports Coordinators utilize a person-centered planning approach and are trained to work collaboratively with individuals, their families, and service providers to develop an Individual Support Plan (ISP) that addresses the specific needs and desired outcomes of the individual. We assist these individuals in locating, coordinating, and monitoring the community resources they need to live the fullest, healthiest lives possible.
JOB OVERVIEW:
The Supports Coordinator locates, coordinates, and monitors services for individuals diagnosed with intellectual and developmental disabilities. The Supports Coordinator's primary responsibility is to implement the services based on the Individualized Service Plan (ISP) for each of the consumers on their caseload.
RESPONSIBILITIES:
* Meet with new consumers and their families as appropriate to explain services, obtain consents, and begin the development of an ISP.
* Submit specific required paperwork and documentation for each client in accordance with regulatory standards as identified by ODP as well as PersonLink internal policies.
* Make referrals to organizations and other appropriate resources to provide services to the consumers.
* Support and empower individuals to participate in their home and community and provide other supportive resources.
* Advocate for consumers and families within and outside the system.
* Facilitate ISP reviews and monitoring activities and meetings in accordance with all regulatory standards and PersonLink policies.
* Attend meetings, trainings, and conferences as required.
* Complete other duties as assigned.
SKILLS:
* Excellent organizational and time management skills
* Excellent verbal and written communication skills
* Excellent interpersonal, analytical, and problem-solving skills
* Good computer skills (ability to type and knowledge of some software programs)
QUALIFICATIONS:
* Bachelor's degree from an accredited college or university required
* Must have completed at least 12+ college credits in one of the following subject areas: Sociology, Social Welfare, Social Work, Anthropology, Psychology, Counseling, Gerontology, Administration of Justice, Criminal Justice, Criminology, Human Behavior, Human Services, Human Development, Special Education, Economics, Geography, History, Political Science
* Preferred: 1+ years of experience in case management
SALARY GRADE: 15
