PURPOSE OF THE POSITION: To provide supervision of programs both on site and in the field for multiple HUB sites in Long Island.

REPORTS TO: Assistant Day Habilitation Director

SUPERVISES: Day Habilitation Specialists

SPECIFIC RESPONSIBILITIES -

  • Interview, screen, and evaluate staff to provide Day Habilitation services.
  • Provide supervision to staff during shift, including assigning responsibilities, changing responsibilities and/or assigned individuals as needed.
  • Ensure program operates in accordance with pertinent regulations and agency policy and procedure.
  • Ensure all individuals' treatment plans are followed appropriately.
  • Attend and contribute to Critical Behavior Review meetings as it pertains to individuals attending the program.
  • Provide direct support services as needed.
  • Provide direct observation of staff implementing staff actions
  • Monitor individuals' health and safety throughout the shift.
  • Ensure that planned activities for community integration and/or recreation are completed and documented appropriately. Create and distribute weekly calendar of activities.
  • Ensure completion of all documentation in accordance with regulations and agency procedures.
  • Ensure that all prescribed reporting procedures are followed accurately and within appropriate time frames.
  • Ensure all regulations, agency procedures, and notifications are followed in regard to incident management. Ensure completion of appropriate documentation of same.
  • Ensure that the daily schedule and routines are followed.
  • Provide support to staff at multiple HUB sites on an on-going basis, including potential and actual crises.
  • Assist in training staff in implementation of individuals' treatment plans.
  • Model calmness and assurance in order to set a tone of stability and control in daily activities as well as in crisis situations for both individuals served and staff.
  • Model appropriate interactions with individuals, staff, family members, community members, etc. at all times
  • Attend program individual screenings and Life Plan meetings (every six months)
  • Write Staff Action Plans and ensure documentation is completed and submitted in a timely manner.
  • Review of daily and monthly summaries
  • Work hand-in-hand with staff and individuals in the field during program hours as needed.
  • Within 60 days of hire obtain and maintain AMAP, SCIP-R, CPR and First Aid certifications.
  • Conduct initial and annual evaluations of staff.
  • Maintain census to optimize funding.
  • Encourage staff career development through LMS trainings
  • Work with Assistant Director on development, training and rollout of electronic health record.
  • All other duties requested by the Assistant Director or administration.
  1. QUALIFICATIONS -
  • High School Diploma required; Associates/Bachelor's preferred.
  • Comprehensive knowledge of OPWDD regulations
  • Good management, organizational, and interpersonal skills
  • Good oral and written communication skills
  • Minimum of 2 years' experience in programs serving individuals with developmental disabilities
  • Computer literate.

III. RELATIONSHIP WITH OTHERS -Provide supervision in a positive and supportive manner. Maintain positive relationships with other departments and families both in and outside of SCO.

In addition, must possess the following characteristics:
  • Be a team player.
  • Have a strong sensitivity to cultural differences present among staff and individuals within the organization. Ensure respect, diversity and inclusivity of the staff working in the day hab hub with SCO.
  • Possess a strong belief in people's ability to grow and change; forge a mutually respectful partnership with persons served and their families.
  • Be able to set limits and maintain helping role of practitioner and to intervene appropriately.
  1. WORKING CONDITIONS - 35 hour/week, both on site and in the field.
