GENERAL FUNCTION: Provision of direct casework to foster and former foster youth and support with daily operations and outreach for the Transitional Housing Program.

QUALIFICATIONS:

+ Must be able to be associated with Casa de Amparo through Community Care Licensing (CCL), which typically includes clearance by FBI, DOJ and CACI.

+ Must be able to respond to any on calls/emergencies at New Directions office in Oceanside in under 40 minutes.

+ Bachelor's degree in a behavioral science or bachelor's degree and a minimum of two years' experience in a public or private child welfare social services setting, or specific experience working with transition age youth who are 16 to 24 years of age.

+ Strong understanding of transition age youth (TAY) development, the effects of trauma, California Title 22 regulations and San Diego County private/public social service agencies.

+ Proficient in English and have clear written and verbal communication skills.

+ Strong time-management, organizational, interpersonal and interviewing skills.

+ Ability to comply with flexible schedule to meet participant needs is required.

+ Competency in MS Office and Outlook.

+ Must have clear DMV report, reliable transportation with auto insurance, and be approved to drive by the agency's insurance carrier.

+ Ability to comply with all employment requirements including, but not limited to CPR/Pediatric 1st Aid, physical, TB and drug screen clearance. Must provide proof of immunization against influenza, pertussis, and measles.

+ Must have the physical ability to participate in activities and perform related physical skills, including, but not limited to, crouching, lifting 50 pounds, crawling on hands and knees, and walking up and down stairs.

MAJOR JOB SEGMENTS

Program:

+ Deliver direct case management services to program participants.

+ Deliver group education and support to program participants in monthly groups and/or make connections and arrangements for guest educators.

+ Deliver individual education and support to participants as a follow-up to group curriculum for youth needing intensive education and support in specific areas.

+ Adhere to the county contract requirements and Title 22 regulations.

+ Collect data required for county Monthly Progress Report for participants on caseload and provide to Program Manager and/or Director of Transitional Housing.

+ Assist Program Manager and/or Director of Transitional Housing with required outcome monitoring.

+ Assist Program Manager and/or Director of Transitional Housing with ETO database entry and reports.

+ Assist Program Manager and/or Director of Transitional Housing with John Burton Advocates for Youth (JBAY) database entry and reports.

+ Assist Program Manager and/or Director of Transitional Housing with linkages to outside educational, financial, health, mental health and employment resources.

+ Provide outreach and presentations to referral sources regarding program services.

+ Initiate connections with outside educational, financial support, health, community and employment resources to facilitate delivery of services.

+ Facilitate development and maintenance of community partnerships.

+ Share information with program staff and participants on community resources.

+ Interface with maintenance staff and apartment management staff.

+ Oversee community service activities of participants.

+ Provides initial and ongoing training to mentor volunteers.

+ Refer and interface with participant's mental health professionals, as needed.

+ Complete case plan follow-up activities with participants.

+ Serve as alternate Housing Specialist during staff absence/vacations.

+ Assist with roommate mediation and education to resolve or prevent current or future roommate issues.

+ Rotate being the on-call staff 24/7 for 7 days in a row with Advocates and Program Manager and/or Director of Transitional Housing.

+ Meet with each assigned client weekly at minimum and enter each assigned client's apartment weekly at minimum, unless otherwise stated in an approved Behavioral Health Assessment (BHA).

+ Complete all intake assessments and required documents by determined deadlines.

+ Maintain written documentation, including client case notes, at the clinical standard of the field and enter all documentation into ETO weekly.

+ If applicable to education and experience, provide individual and/or counseling services to participants.

+ Assist with apartment moves and cleaning of apartments as needed.

Fiscal Management:

+ Responsible for remaining within determined budget guidelines.

+ Maintain cash and supply controls.

+ Submit expense reports and mileage expense reports to Program Manager and/or Director of Transitional Housing in a timely manner.

Other:

+ Collaborate with supervisor to assure delivery of casework services in compliance with contract, including reports and database input.

+ Attend supervision and management meetings as assigned.

+ Participate in overall program development, strategies for growth and evaluation.

+ Assure that volunteer resources are used effectively within the program.

+ Implement policies, goals, priorities, standards, and procedures as developed by staff and approved by the Board of Directors.

EFFECT ON END RESULTS: This position assures the professional, thorough and efficient, case management and office functions in support of the Transitional Housing Program.

SALARY OPPORTUNITY : $70,119.05 full-time, exempt
