Position SummaryThe primary function of this role is to recommend and facilitate access to various services, including medical, educational, social, vocational, and housing resources, to achieve program objectives. This position involves initiating referrals for advanced care, collaborating with healthcare providers for comprehensive care, and advocating for clients' best interests.
Key ResponsibilitiesAccommodations may be provided for individuals with disabilities to perform essential job functions.
- Deliver case management services that include crisis intervention, discharge planning, individual and group counseling, mental health and substance use disorder support, screening, and assistance with basic needs and symptom management.
- Ensure accurate and timely documentation while meeting minimum billing requirements for direct client services.
- Facilitate client placements, including pre-placement visits and necessary transportation.
- Participate in shift coverage, including being part of the on-call team.
- Provide field-based services as dictated by program and client needs.
- Uphold client confidentiality in all aspects of care.
- Engage in scheduled program meetings.
- Perform other relevant duties as assigned.
- A Master's degree or a Bachelor's degree with four years of experience in social services is required. Acceptable qualifications include ASW, MSW, MFT, LMFT, LPCC, or APCC.
- For Substance Use Disorder Case Management, certification from a recognized board is preferred, and registration is required.
- A valid driver's license and current auto insurance are necessary.
- Experience in providing mental health and/or substance use disorder services is preferred.
- Cultural competency is essential.
- Bilingual candidates are preferred.
- Familiarity with Microsoft Office and EHR software is advantageous.
