About This Hospice Response Team Licensed Practical Nurse (LPN) Position

HopeHealth is seeking a Hospice Field LPN for its affiliate HopeHealth Hospice & Palliative Care.

The Response Team LPN will respond to urgent patient needs and require excellent clinical assessment skills and an ability to quickly identify the best course of action to treat patients. This requires a LPN who is comfortable in an action-oriented role and who is comfortable assessing and treating a patient, then moving on to the next need.

This is a full-time 40 hour night position that includes a weekend and holiday rotation of every-other weekend, (1) major and (1) minor holiday.

The hours for this position are 11:00am-7:30pm and is eligible for benefits. This position is eligible for a sign on bonus up to $2,500.

What will you do as a Hospice Response Team Licensed Practical Nurse (LPN)?

* Implements the individualized plan of care and recommending revisions to the plan as necessary and plans for discharge/transfer as appropriate.
* Consults with and educates the patient/family/caregiver regarding the disease process and teaches the patient/family/caregiver self-care techniques, as appropriate. Provides medication, diet and other instructions as ordered by the physician.
* Initiates appropriate preventive and rehabilitative nursing procedures.
* Coordinates all patient/family services and prioritization of needs with the members of the interdisciplinary team to develop and revise patient's individualized plan of care.
* Informs the physician and other personnel of changes in the patient's needs and outcomes of intervention.
* Determines scope and frequency of services needed based on acuity and patient/family needs.
* Coordinates care with other interdisciplinary team members in cooperation with the family/caregiver to meet identified needs of the patient/family/caregiver; including psychosocial, spiritual, and emotional needs and refers to appropriate disciplines/services.
