Clatsop Behavioral HealthCare's North Coast Crisis Respite Center (NCCRC) in Warrenton, Oregon is looking for a dedicated service provider to fill a full-time day shift position as Residential Support Specialist with QMHA certification. Under the direction of the North Coast Crisis Respite Manager, the Residential Support Specialists work as a member of a team working to make a difference in the lives of persons with mental illness. This position involves participation and implementation of program goals and objectives to promote a strengths-based, client centered approach to assisting residents in achieving individual goals and ultimately making the leap to independence.
NOC shift 7pm-7am with $2.00 shift differential.
Essential Duties and Responsibilities:
* Maintain and document service notes
* Provide case management and document progress.
* Encourage and motivate residents to engage in new experiences and accomplish personal goals
* Safely transport residents to appointments and community outings
* Ability to facilitate groups and work individually to skills train
* Ability to distribute and document medications accurately
* Cooking and cleaning
Requirements:
* Ability to work any shift
* QMHA certification or ability to get a certification within 6 months of hire. QMHA certification must meet the following minimum qualifications:(a) Bachelor's degree in a behavioral science field; or (b) A combination of at least three years of relevant work, education, training, or experience.
* Valid Oregon or Washington Driver's license or ability to obtain by date of hire
* Motor Vehicle Record clearance that meets agency insurance requirements
* Ability to pass DHS background check clearance
Preferred:
* 3 years prior experience in same or similar field
* Some higher education or vocational training in related field
* Trauma informed care, behavioral health or mental health training or experience
* Bachelor's degree preferred

Developmental Disabilities Service Coordinator ARPA
This position qualifies for a hire-on bonus of $1000, with half paid with the first check after hire and half paid with the first check following a 6-month probationary period.
This position enhances CDDP services and advocates for individuals in Developmental Disability (DD) Program. The Service Coordinator will ensure that clients receive the most suitable services available. Work with a diverse population with specific support needs. Identify the individual's strengths and challenges to provide support in a safe manner. Conduct quarterly monitoring of support being provided in foster homes and group homes. Conduct annual visits to the homes of individuals receiving in-home support.
Essential Duties and Responsibilities:
* Develop ISPs (Individual Support Plans) and all related documentation. This includes, but is not limited to, Risk Identification Tracking forms, Person-Centered Information Forms, and Related support documents.
* Conduct and/or participate in needs assessments (Oregon Needs Assessments, Child Needs Assessments, Adult Needs Assessments, Support Needs Assessment Profile). All assessments are conducted and/or reviewed by the Service Coordinator.
* Provide Choice Counseling to ensure all available choices/options are explored. Assist in coordinating chosen services.
* Monitor services, client records, and documentation in group homes, foster homes, and/or supported individuals' own homes to verify standards are being met, based on the Oregon Administrative Rules, including Medication Administration Record (MAR) Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), finances, safety, support plans being followed, implementation of Behavior Support Plans, Incident Reports, Protocols, Fire Evacuation Records, Behavior Data, etc.
* Facilitate team meetings for each client at least annually and as needed.
* Complete Case Management Contacts as dictated in the ISP.
* Document all client contacts/maintain current, accurate PNs (Progress Notes) and bill for services.
* Identify housing for individuals seeking residential services outside of their family home. This may include children and adult foster homes, group homes, out-of-county placement, and supported living. Services are then developed based on client desires, needs, and interests.
* Complete annual Level of Care for K-Plan and Waiver services
* Complete Plan of Care in eXPRS to reflect and approve funding.
* Approve Personal Support Worker timesheets in eXPRS.
* Complete Notice of Planned Action and assist individual or family to file hearing request, if desired.
* Attend required meetings and trainings.
* Advocate for clients' rights and services.
* Collaborate with community providers.
* All CBH Staff are Mandatory Abuse Reporters per the State of Oregon, and as such all staff must be aware of and follow Mandatory Abuse Reporting policies.
* Other Duties as Assigned
Required Education and Experience:
* Knowledge of the public service system for Developmental Disability services in Oregon.
* Must have, at a minimum, a Bachelor's Degree - in Behavioral Science, Social Science, or a closely related field; Or a Bachelor's Degree in any field AND one year of human services related experience; Or an Associate's Degree in Behavioral Science, Social Science, or a closely related field AND two years human services related experience; or three years of human services related experience.
* Skilled at working as a member of an interdisciplinary team and completing required documentation.
* Excellent oral and written communication skills. Full professional fluency in English.
* Strong attention to detail, highly organized and ability to multitask from anywhere in the service region.
* Articulate well, speak precisely, and think and write clearly.
* Required to have and maintain an Oregon or Washington Driver's license and meet the agency's insurance carrier's insurance standards at the time of hire.
Desired Skills:
* Knowledge of the Health Insurance Portability and Accountability Act (HIPAA) and the importance of maintaining confidentiality.
* Skilled at remaining calm in stressful situations and de-escalating potential aggression from clients.

Mobile Crisis Clinician, QMHP (on-call)
This position qualifies for a hire-on bonus of $5000 with 1/2 paid with 1st check after hire and 1/2 paid with 1st check following 6-month probationary period.
Clatsop Behavioral Healthcare (CBH) has an immediate opening for an on-call Mobile Crisis Clinician, QMHP. This position will provide Crisis Assessment and Intervention, follow-up support, and other outreach for individuals experiencing crises in a variety of settings, including local hospitals, jails, and diverse locations within the community. The Mobile Crisis Team works closely with Law Enforcement and other community partners to resolve crises in the natural setting in which they occur in order to defer from a higher level of care if possible.
Requirements:
* Master's Degree in a Behavioral Health-related field and meet qualifications to be a Qualified Mental Health Professional (QMHP) in the state of Oregon.
* Valid driver's license or ability to obtain Oregon or Washington Driver's License
* Background check clearance
* Ability to meet agency insurance requirements must submit current motor vehicle record if licensed outside of the state of Oregon
Preferred:
* Three (3) years of direct service with individuals experiencing severe and persistent mental illness.
