*Responsibilities:*
* Adhering to the Best Practices established by the Board of Nursing and CAHC Accreditation Standards
* Open communication with other Team Members, Clients, and Family
* Provide input regarding placing certified home health aides (CHHAs) appropriately with clients
* Supervision of CHHAs in the greater community
* Making monthly follow-up calls/visits to ensure adherence of Care Plan
* Evaluate effectiveness of care provided by CHHAs with timely completion of 60-day skills assessments during on-site visit

*Requirements:*
* New Jersey Registered Nurse (RN) License, BSN preferred.
* BLS Certification
* New Jersey Driver’s License, and an automobile in good working condition with appropriate insurance coverage in accordance with New Jersey laws.
* Minimum of one-year supervisory experience in health care setting, preferably home care, hospice, or long-term care
* Ability to join on-call rotation

*Skills:*
* Exceptional communication and interpersonal skills
* Robust critical thinking and problem-solving abilities
* Capable of working independently and collaboratively in a multidisciplinary team
* Proficient in electronic medical record systems and relevant software applications
