*Education & Experience:*

* License Practical Nurse (LPN) in the state of Ohio
* Associate or Bachelor of Science Degree in Nursing or in related field
* Must be CPR/First Aid Certified
* Ohio driver’s license, current auto insurance and access to a car during working hours.
* Home Health experience with terminally ill patients preferred.

*Skills & Qualifications:*

* Strong ability to multitask and work in a fast-paced environment
* Excellent skills in leadership, management, interpersonal relations, networking, family, and customer relations.
* Strong verbal and written communication skills.
* Demonstrate flexibility, excellent assessment skills, and ability to supervise others and the confidence to work independently.
* Demonstrate skill in nursing practice, ability to cope with patient/family emotional stress and tolerance of individual lifestyles.
* Active listening skills with the goal of understanding
* Possess skills of working with team having positive outcomes

*Job Duties:*

* Assume responsibility for patients and families under the care of Zusman Community Hospice. May include assessment, planning, implementation and evaluation phases of the nursing process.
* Initiate communication with attending physicians, other hospice team members and agencies as needed to coordinate optimal care and use of resources for the patient/family.
* Maintain up-to-date records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
* Perform other related functions as required by the Director.

Pay: Up to $31.00 per hour
