Care Coordinator

We are looking for an excited team player able to effectively multitask in a fast paced, ever changing environment. You must be a fast learner and very detail oriented. You must also be available to work on-call every other weekend. Responsibilities include but are not limited to:
- Perform duties related to the supervision, instruction and motivation of aides.
- Filling all open shifts with aides based upon the needs of client, employee qualifications, and plan of care.
- Ability to effectively build one-on-one relationships with both clients and caregivers.
- Timely and consistently document all information in contact management system.
- Maintain a current roster of staff, their qualifications/licensures, and their availability.
- Perform support duties for the nurses in-house and in the field.
- Monitoring telephony system to ensure caregivers are arriving to client's homes as scheduled.
- Entering missing times/shifts in a timely fashion.
- Participate in in-service education, quality assessment activities and other staff meetings as required.
- Relay pertinent information about patients to the assigned staff, RN and Manager.
- Maintain an exceptional level of professionalism and responsiveness with prospective clients and others who may communicate with the agency.
- Receiving and reviewing caregiver care notes.
- High school diploma
- Minimum of 1 year experience in personal care, or home health care or related field as care coordinator.
- Supervisory experience of at least 1 year.
- Excellent, professional interpersonal and communication skills - oral, conversational, telephone and written.
- Effective organizational and task management skills.
- Ability to demonstrate good judgment.
- Must work well in fast paced environment and adapt quickly to changing conditions.
- Must be proactive and able to successfully and effectively adhere to tight deadlines.
