Caregiver Support and Coordination Position

We are looking for an experienced and compassionate individual to join our team as a Caregiver Support and Coordination Position. In this role, you will be responsible for providing direct care to clients and coordinating the delivery of home care services to ensure they receive the highest level of support and care.
About the RoleThis is a full-time position offering a competitive salary of $19.00 per hour and a comprehensive benefits package including health insurance, paid sick time, paid time off, and vision insurance. The ideal candidate will have a strong background in care coordination and direct care, excellent communication and interpersonal skills, and the ability to work well in a fast-paced environment.
Key Responsibilities:
- Provide direct care to clients, including assistance with daily living activities;
- Coordinate the delivery of home care services to clients, including scheduling and staffing;
- Monitor and manage client care plans to ensure continuity of care;
- Communicate effectively with clients, caregivers, and other stakeholders to ensure seamless care delivery;
- Provide support to nurses and other staff members as needed;
- Maintain accurate records of client care plans and caregiver qualifications;
The successful candidate will have:
- A high school diploma or equivalent;
- At least one year of experience as a care coordinator in personal care, home health care, or a related field;
- Excellent professional interpersonal and communication skills;
- Effective organizational and task management skills;
- Good judgment and problem-solving abilities;
- The ability to work well in a fast-paced environment and adapt quickly to changing conditions;
