About Blue Circle Health:Blue Circle Health is a 501(c)(3) nonprofit organization dedicated to improving the health of people with type 1 diabetes (T1D), a chronic autoimmune condition in which the body doesn’t produce insulin. T1D affects more than 2 million people in the US, and many face barriers to the care, education, medication, and supplies needed to manage their condition. 
Blue Circle Health provides virtual care, education, and support for adults with T1D, at no cost to them. Because T1D is a 24/7 condition that is largely self-managed, we educate and empower people with T1D to ensure they have the skills, tools, and confidence to manage their condition. Our whole-person approach includes medical care, diabetes education, peer coaching, diabetes supportive counseling, insurance navigation, supplies and medication, and referrals to community resources that can help with access to housing, employment and food.

Funded by the Leona M. and Harry B. Helmsley Charitable Trust, we are building a high-quality virtual healthcare delivery and support model for people with T1D. We partner with patient advocacy groups, health systems and clinics, including Federally Qualified Health Centers, to reach people with T1D. We aim to show that our model can be replicated for other chronic conditions to improve health outcomes and reduce the burden on the US healthcare system.

About the Role:The Clinical Assistant plays a pivotal role in our virtual diabetes care program, ensuring seamless patient visits and equipping providers with essential information for effective consultations. We’re looking for a compassionate self-starter who enjoys building new processes that directly benefit our patients. 
This is a full-time, home-based position, eligible for a full benefits package including paid holidays and time off, and competitive pay. A private, quiet, home office space is required for patient appointments.  As a remote organization, care is provided to patients via phone, SMS, email and video. Our hours of operation are 8:00am - 8:00pm EST, Monday-Fridays. The schedule for this role is 8-5 EST, with a lunch break. This is a full-time position, 40 hours per week.

Core Responsibilities:
  • Conducts pre-clinical calls to obtain medical history, reviews medications and confirms each patient has their telemedicine link in advance of each clinic appointment to ensure that the patient is ready and waiting for each visit.
  • Answers the BCH phone lines, responds to emails and triages calls following the escalation protocol. Reviews and triage incoming faxes and emails.
  • Obtains medical records from external providers.
  • Proactive in calling pharmacies to confirm receipt of medication prescriptions. 
  • Ensures that prescriptions are up to date and may provide backup support for prior authorizations.
  • Acts as a liaison between patient and other clinicians, including outside providers.
  • Schedules patient appointments with appropriate service lines by following the patient’s individualized care plan. 
  • Assists in setup and syncing of diabetes devices to respective online portals to ensure smooth provision of care with the medical team.
  • Cross trained to assist as back up to Clinical Enrollment Specialist with patient enrollment.

Required Credentials, Experiences and Capabilities:
  • Fluency in Spanish and English required. 
  • Minimum of 1 year of experience in the medical field, clinic or office required. Experience with telehealth or remote work is preferred. 
  • Must have a clinical license or certification: MA, EMT, LPN, or RN. Other licenses are considered on a case by case basis. Wage will depend on experience and license. 
  • High level of digital literacy, and the ability to train basic technology skills to patients who may have no experience with technology. 
  • Experience with type 1 diabetes preferred. High level of flexibility and the ability to adjust as the organization grows.
  • Strong interpersonal and communication skills and the ability to work effectively, remotely, with people of diverse cultures, ages, and economic backgrounds in a culturally competent and sensitive manner. 
  • Ability to provide high quality care in a virtual environment.
  • Ability to listen actively, communicate with empathy and gather information in a respectful manner. 
  • Ability to use independent judgment and analyze information from multiple sources. 
  • Ability to communicate information to medical professionals, clinical employees, coaches, insurance companies, and other community organizations over the phone or virtually.
  • A willingness to accept feedback to ensure provision of high quality care for people with T1D.
