Job Summary:

Rosalie’s Place is a transitional housing facility that provides working or ready-to-work adults with support with accessing employment and temporary bridge from homelessness to permanent homes. Reporting to the Rosalie’s Place Program Manager, the intake and exit coordinator oversees our program waiting list and entry processes, assists in coordinating move outs, is responsible for follow up services after exit, provides supportive case management to a limited caseload of workplace ready adults, and supports the case management team with project-based coordination.

The ideal candidate enjoys working as part of a team of staff and volunteers. This candidate can think critically to determine the best fit for clients and works always toward the best interest of the client and the program. This individual can work independently while ensuring complete and thorough communication with team members. The best candidate for this post is very organized, detail oriented, comfortable with creating lines of communication with community programs, and able to encourage and support follow through from residents. This representative must be energetic, resourceful, and reliable with strong interpersonal skills and a passion for working with vulnerable populations. This person will exhibit the demonstrated ability and desire to maintain and promote an organized, safe, and efficient work environment. This candidate must be able to build relationships with both clients and volunteers in order to conduct home visits in a safe and thoughtful manner.

Specific Duties:

  • Oversees intake processes at Rosalie’s Place, including monitoring wait list, conducting screenings for applicants, scheduling intakes, and arranging for residents to move in.
  • Completes intake assessments and assigns intakes to interns and staff.
  • Ensures completeness and eligibility of applications.
  • Organizes and maintains paperwork for intake related processes.
  • Oversees front desk operations, including ensuring phone coverage.
  • Assists case managers in coordinating exits to permanent housing, including assisting with obtaining furniture, scheduling movers, and locating resources for exiting clients.
  • In conjunction with case managers, maintains contact with former residents to monitor employment & housing retention.
  • Coordinates and conducts home visits as needed with other staff or volunteers.
  • Provides limited workforce case management to residents living in our sister transitional housing facilities, Ozanam Manor and DePaul Manor.
  • Oversees special projects as assigned by the Program Manager in support of the program.
  • Other duties as assigned by the Program Manager
  • Participate in and serve as a model for SVdP’s Culture of Philanthropy, and ensure SVdP volunteers feel welcomed and appreciated *

Required Skills/Abilities:

  • Level One Fingerprint Clearance or ability to obtain within 90 days of employment.
  • CPR and First Aid certification or ability to obtain within 90 days of employment.
  • Excellent communication, organization skills, and ability to interact with residents, staff, and volunteers required.
  • Ability to support clients with resume writing, soft-skills enhancement, and job search skills.
  • Related experience working with vulnerable populations, volunteers, and staff in a temporary shelter environment preferred.
  • Must have valid driver’s license with a clean driving record.

Preferred Skills/Abilities:

  • Knowledge of available community support and services is beneficial and will be shown preference.
  • Strong computer skill, proficiency with Microsoft Suite programs, and excellent electronic communication and documentation preferred.

Education and Experience:

  • BA/BS in social work or medical field preferred, or equivalent experience
  • Proficient in standard computer systems (Microsoft Office)

Physical Requirements:

  • Abilities to stand and walk, lift up to 50 pounds, sit, bend, and reach are required for this position.
  • Ability to read, comprehend, and communicate orally and in writing required.
