Care Coordination Associate, LPN

The Care Coordination Associate (LPN) position is primarily responsible for assisting the Referrals and Admissions Department in enrolling patients into agency services. This position requires knowledge in all agency programs including hospice, palliative care, Staywell, and other programs offered by Hospice of the Piedmont. The Care Coordination Associate participates in various parts of the admissions process including but not limited to: conducts patient visits across various settings, assists with collecting patient information, schedules admissions, and enters patient information into the record. This role will assist the department in various capacities as deemed necessary.
Essential Duties
- Assists with referral intake process by providing program information to prospective patients, families, and referral sources. Provides information, collects information, and develops the referral according to agency procedure. May be involved in the election statement signing.
- Evaluates patient’s appropriateness and eligibility for program services. Communicates with Director of Referrals and Admissions regarding assessment findings. LPN will communicate with other members of the interdisciplinary team in order to introduce the patient.
- Completes accurate and timely documentation of communication with patient/family and other medical personnel involved in the case according to the policies of Hospice of the Piedmont. Documents same in acute care facility’s chart according to procedures arranged with that facility.
- Acts in a professional manner to collaborate with physicians, other health care providers, and interdisciplinary team members and makes timely reports of pertinent information to appropriate health care professionals.
- Obtains and gathers relevant medical information related to the patient. Requests medical records from physicians, hospitals, long-term care facilities, and other healthcare entities to support the Referrals and Admissions team in determining eligibility of the patient.
- Inputs information into our electronic medical record including a referral note about the patient. Other required information includes but is not limited to: including basic demographic information, insurance and payer sources, admission and status referral codes, and order to admit from MD.
