ACLT Benu Community Home seeks a culturally responsive and trauma-informed licensed clinical social worker to serve as the Sr. Case Manager leading the Case Management Team. This Sr. Case Manager is a leadership position that will foster a team environment with ACLT's Benu Community Home employees on the case management and advocate team. This position will also provide supportive services related to mental health, behavioral health, and substance use disorder to guests of the Benu Community home. Benu Community Home is committed to making a transformational impact, working with guests to assist them in achieving self-sufficiency and permanent housing.

ABOUT THE POSITION

The Sr. Case Manager supervises the Case Management team working with Black men transitioning from chronic homelessness who may have significant mental or physical health issues and/or substance use disorders. Benu is committed to providing a positive harm reduction approach, emphasizing creating a culturally responsive environment of success, meeting individuals "where they are at," and providing non-judgmental, non-coercive services and resources to individuals. The Sr. Case Manager will work directly with the Case Management team and community guests to assist them with clinical services as the case manager works to obtain permanent housing by developing individualized goals for enrichment, stability, wellness, and recovery.


Typical Duties and Responsibilities:


Clinical

  • Provide culturally responsive cognitive behavior therapy (CBT) counseling to guests at both the individual and group levels around emotional and mental health issues.
  • Provide evaluations and assessments for the purpose of referral to treatment facilities and other community resources to address specific guest situations.
  • Build and maintain strong rapport with clients both in groups and on an individual basis.
  • Ability to provide accurate clinical assessments of guests at Benu Community Home, which can include mental and emotional health and stability, as well as generalize physical assessments when dealing with guests in the grip of substance abuse.
  • Maintain detailed and concise records of each client throughout their treatment, whether at the client's behest or as part of a treatment program that may be court-ordered.
  • Ability to perform professional consultations with medical professionals, therapists, physicians, and even officers of the courts where appropriate
  • Perform administrative duties as needed to respond to other social services organizations or government agencies related to the guest's needs.
  • Responding to crises to deescalate guests in Benu Community Home.
  • Serve as a site preceptor for students enrolled in social work degree programs, as possible.
  • Other Duties as assigned.

People Management

  • Provide supervision to four case managers responsible for guests screening, intake, data collection, program workshop, and group facilitation, and weekly meetings with guests assigned to their caseloads.
  • Ensure staffing coverage for a 24-hour enhanced living community
  • Assist with training and capacity building for case management to stay abreast of best practices for culturally responsive care models.
  • Meet regularly with each case manager to review strategic program goals and objectives and provide coaching.
  • Maintain accurate records of direct reports' hours worked and ensure accurate timesheets. T

Supervise Case Management

  • Guide case managers on the best approach to working with community guests to identify specific barriers they face to participating in goal achievement and developing the life planning skills needed to move into appropriate housing.
  • Build rapport, provide support, and advocate for a diverse range of community residents, including individuals who are actively using substances and/or are living with significant mental/physical health challenges using harm reduction approaches when applicable.
  • Participate in verbal de-escalation and physical interventions in emergent situations, and be willing and able to assist other staff in maintaining a safe and secure environment. Mediate disputes between community guests.
  • Provide training to case managers and advocates on best practices for mitigating Guests' behaviors that could adversely affect their ability to continue in the program.
  • Provide guidance to case managers on resources for community guests to obtain basic housing documentation and, when appropriate, delegate a case manager to accompany community residents to outside appointments.
  • Ensure ongoing engagement and meetings with community guests occur and progress is being tracked toward goals.
  • Ensure the housing case manager develops relationships with landlords, referral services, treatment providers, and other potential community-based partners and resources to support guest success. e
  • Ensure the case management team is current with guest records, case notes, and HMIS data entry.

Teamwork

  • Attend staff meetings, workshops, retreats, community meetings, and in-service training as provided.
  • Contribute to and support a positive, team-oriented work environment; participate in the group decision-making process with other staff members.
  • Collaborate with the Program Coordinator and Director of Programs and Services to ensure program alignment with performance management expectations.
  • Collaborate with the Development and Communication team on opportunities to obtain program resources and funding for Benu events and program needs.
  • Other Duties as assigned.

JOB CONDITIONS:

This position requires the employee to work in an environment where exposure to blood, bodily fluids, and other potentially infectious material may occur. Other exposures could include cleaning supplies, chemicals involved in pest control, paint, and other materials used in building maintenance. Working conditions may include interruptions, working alone, interactions with angry persons, and exposure to computer CRTs.


MINIMUM QUALIFICATIONS

  • Licensed Clinical Social Worker with a master's degree in social work from an accredited academic institution.
  • Two years or more experience performing clinical work in a housing program or community-based healthcare or living setting providing services to similar demographics.
  • Experience delivering culturally competent services and working effectively with multicultural demographics, specifically Black men.
  • Strong knowledge of problems individuals face, including alcohol and drug abuse, mental illness, trauma, domestic violence, and chronic homelessness (houselessness).
  • Knowledge of various therapy options and treatments for men, specifically Black men experiencing trauma.
  • Solid demonstration of empathy and emotional intelligence.
  • Committed to supporting and upholding ACLT's mission, beliefs, and values.
  • Familiarity with local resources that could help individuals and families
  • Problem-solving and critical-thinking skills to provide creative solutions and adaptive strategies to meet the adaptive challenges of this unique demographic served.
  • Organizational and case management skills and experience.
  • Ability to collaborate and work well as part of a team
  • Proficient computer skills and ability to use Office 365, Google Docs, Databases, Shift Notes, or similar computer or cloud-based programs or electronic medical records
  • Willingness to learn and work within a diverse environment.
  • Familiarity with and/or willingness to learn best practices and successful approaches to responding to the unique lived experiences of African American men.
  • Commitment to harm reduction program model.
  • Experience successfully working with ambiguity in a fast-paced environment.
  • Experience with data collection, presentation, and report writing.

PREFERRED QUALIFICATIONS:

  • Knowledge of the local subsidized housing and low-rent private housing market.
  • Experience with chronically homeless adults.
  • Proficiency in a secondary language.
  • CPR / First Aid training.
