The Roadmap to Reunification (R2R) / HOPE (Housing Opportunities to Personally Excel) specialist demonstrates competence and is experienced in working with NH DCYF, DCYF involved families and caregivers, collateral service providers, and young adults ages 18-21. The R2R/HOPE specialist ensures compliance with all meeting and paperwork deadlines as outlined by the R2R contract and specializes in working to stabilize young adults who are DCYF involved between the ages of 18-21.

This is a Full Time, 40-hour per week position.

Responsibilities include, but are not limited to:

  • Carries a caseload as indicated including R2R and HOPE cases

  • Completes all required documentation and R2R meetings per contract specifications

  • Maintains contact with individuals, families, caretakers and funders as outlined in program

  • Always maintains appropriate boundaries and a high level of professionalism with individuals, families, funders, collaterals and colleagues

  • Adheres to all agency policies and procedures

  • Produces documentation and tracking under direction of their supervisor, DCYF, or other agency standards

  • Participates in on-call rotation

  • Ensure that a minimum of weekly check-ins occur with individuals, biological families and placement settings for each case assigned as applicable

  • Ensure team meetings are held a minimum of every 30 days to review action plans, progress towards goals, identification of barriers to achieving goals, and outlining measurable tasks for all cases assigned

  • Provide creative solutions to any barriers to achieving case goals or reunification

  • Maintains communication with referral sources and other service providers as needed

  • Assists in scheduling appointments with collateral agencies as needed

  • Reports to Supervisor when issues are not resolved and follows up until the issue is resolved or Supervisor indicates they will take over communication/follow up

  • Engages in a minimum of weekly supervision with supervisor and raises any case-related concerns/needs to supervisor

  • Participate in Clinical Team Meeting and other meetings as assigned

  • Responsible for providing consultative support to R2R specialists to respond to client needs.

  • Responsible for networking with community parents and resources to develop tools to respond to client needs.

  • Provision of in-service trainings as requested to DCYF DOs on service model

  • Compiles monthly data reports as assigned

Qualifications:

  • Bachelor's degree with a major in social work, counseling, psychology, or a related field with an emphasis in human services

  • Have a minimum of two (2) years of experience in identifying barriers and resources to reunification with families

  • Demonstrated knowledge of, needs of children and families in the child welfare system.

  • Valid driver’s license with reliable and consistent access to a vehicle

OR

  • An Associate's degree with a major in social work, counseling, psychology, or a related field with an emphasis in human services

  • Have a minimum of four (4) years of experience in identifying barriers and resources to reunification with families

  • Demonstrated knowledge of, needs of children and families in the child welfare system.

  • Valid driver’s license with reliable and consistent access to a vehicle
