Reunification Support Specialist

Northeast Family Services is in search of a compassionate and skilled Roadmap to Reunification Clinician (R2R) to enhance our dedicated team in New Hampshire.

Role Responsibilities- Act as a liaison and facilitator among the clinical team, clients, families, and all relevant service providers throughout New Hampshire.
- Participate in in-person meetings to enhance care coordination.
- Maintain documentation in accordance with New Hampshire state regulations.
- Engage with treatment teams and caregivers regularly to ensure effective implementation of treatment plans, address specific target areas, and review overall progress toward goals.
- Guarantee compliance, accuracy, and quality of clinical records and documentation.
- Adhere to Federal, State, and agency guidelines related to administrative and clinical policies.
- Ensure that clients receive high-quality and professional services.
- Address parent, caregiver, and family concerns, providing appropriate resolutions.
- Participate in Clinical Team Meetings and other assigned meetings.
- Master's Degree in Counseling, Social Work, Psychology, or a related field.
- A minimum of 1 year of experience working with children and adolescents, identifying barriers and resources for family reunification.
- Proven understanding of the needs of children and families within the child welfare system.
- Valid driver's license with dependable access to a vehicle.
