The Roadmap to Reunification Clinician (R2R) acts as a key facilitator, ensuring smooth connections and comprehensive care for families and children navigating various systems across New Hampshire. This role emphasizes service coordination, uniting the clinical team, NH DCYF, birth parents, caregivers, and all related service providers to enhance treatment services and documentation.

Position Type: Full Time, 40 hours per week

Key Responsibilities:
  • Serve as a vital liaison and facilitator among the clinical team, clients, families, and collateral service providers throughout New Hampshire.
  • Participate in in-person meetings to bolster care coordination.
  • Ensure documentation adheres to New Hampshire state regulations.
  • Engage with treatment teams and caregivers regularly to guarantee effective implementation of treatment plans, monitor progress towards goals, and adjust plans as necessary.
  • Maintain compliance, accuracy, and quality in documentation and clinical records.
  • Adhere to Federal, State, and agency guidelines concerning administrative and clinical policies.
  • Deliver high-quality and professional services to clients.
  • Address parent/caregiver/family concerns and identify suitable resolutions.
  • Contribute to Clinical Team Meetings and other assigned meetings.
Qualifications:
  • Master's Degree in Counseling, Social Work, Psychology, or a related field.
  • A minimum of 1 year of experience working with children and adolescents, identifying barriers and resources for family reunification.
  • Proven understanding of the needs of children and families within the child welfare system.
  • Valid driver's license with dependable access to a vehicle.
