ameriCARE Weatherford is a growing Homecare Agency that is looking for someone who is dedicated, driven and compassionate to fill one of our Assistant Quality Care Coordinators position. 

The Assistant Quality Care Coordinator will work closely with the Quality Care Coordinator to ensure that every caregiver is fully prepared and supported in their role, with a strong focus on client care and satisfaction. This position offers an opportunity to grow with a dynamic team dedicated to making a difference in the lives of our clients.

 

Overview:
The Assistant Quality Care Coordinator plays a crucial role in ensuring the delivery of high-quality home care services. This role demands flexibility and the ability to respond swiftly and effectively to emergency situations in a fast-paced environment. The Assistant Quality Care Coordinator will support the Quality Care Coordinator in various duties, focusing on client satisfaction, quality assurance, and staff management. The role involves administrative tasks, on-call rotations, and direct interaction with clients and caregivers.

Responsibilities:

  • Required to fill open shifts due to callouts or other unforeseen circumstances, when needed.
  • Ensure the satisfaction of clients and their families through regular communication and quality assurance visits.
  • Conduct supervised visits, quality checks, and client/caregiver introductions to ensure the highest standards of care.
  • Collaborate with the Quality Care Coordinator to create daily activity lists and review client and caregiver visits.
  • Participate in weekend on-call and after-hours rotations, providing timely responses to urgent needs.
  • Perform various administrative tasks, including completing documentation and required paperwork, and using computer and app-based tools.
  • Support staff training, provide coaching, and ensure that caregivers have the skills necessary to meet client needs.
  • Understand and accurately interpret Texas Health and Human Services regulations, company policies and procedures ensuring compliance is always met by caregivers.
  • Assist in marketing and relationship-building efforts as needed to support company growth.
  • Participate in the Quality Assessment and Performance Improvement (QAPI) program.

 

Requirements:

  • Certified Nurse Assistant (CNA) certification is required.
  • College degree in a health-related field is preferred.
  • Minimum of 3 years experience in home care, home health, hospice, healthcare, or social services.
  • Must have flexible availability for daytime, nighttime, and weekend shifts, including on-call duties.
  • Reliable transportation is required.
  • Strong ability to lead, give direction, teach, and coach caregivers.
  • Ability to work independently with minimal supervision while maintaining high standards of care.
  • Must have strong organizational skills, attention to detail, and the ability to manage extensive documentation in a fast-paced role.
  • Excellent customer service skills, with a focus on client satisfaction and quality care.
  • Exceptional written and verbal communication skills are essential.
  • Must be proficient with computers and familiar with various apps and software programs used in home care settings.
