Duties and Responsibilities:In this position, you will:
- Exhibit **outstanding customer service** skills while interacting with clients and team members, ensuring a respectful and courteous environment.
- Review care plans established by the case manager before initiating visits, ensuring accurate record-keeping and fulfilling all assigned tasks.
- Respond promptly to inquiries from the Care Services Center regarding scheduling and updates.
- Maintain open lines of communication with your Care Designer to enhance client experiences and care quality.
- Deliver thorough and professional nursing care aligned with care plans, nursing assessments, and client needs.
- Perform nursing skills, treatments, and procedures competently, adhering to the provincial scope of practice and organizational policies.
- Provide education and information to clients and their families regarding diagnoses and care.
- Assist clients with daily living activities, including hygiene, bathing, grooming, and linen changes.
The ideal candidate will possess:
- A current Certificate of Competence and be in good standing with the provincial College of Nurses.
- Graduation from an accredited nursing program and at least one year of relevant experience.
- The ability to function within the limits of personal experience and knowledge, practicing safely and competently.
- Preferred experience in community, home health, or medical/surgical settings.
- A valid Criminal Record Check with a Vulnerable Sector Search, updated every five years at the nurse's expense.
- A negative TB test or chest x-ray, also updated every five years at the nurse's expense.
Flexibility is essential for this role, with options for casual, part-time, or full-time work.
Business Relationships:You will report directly to the Care Designer and collaborate closely with the Client Care Ambassador.
Key Metrics:Your performance will be evaluated based on:
- Attendance and reliability.
- Utilization of the Attendance Monitoring System (AlayaCare).
