Probation: 90 days

We’re also one of the few Federally Qualified Health Centers in the nation with providers who specialize in primary care for LGBTQ people and with a research team working to advance the care and treatment of people in our community.

JOB SUMMARY: Under supervision of the Manager of Housing Case Management, the Housing Stabilization Specialist I for the Intensive Case Management Services (ICMS) program is responsible for meeting contract requirements while assisting clients with supportive services with a client-centric approach. The ideal candidate possesses excellent skills that are sensitive to the challenges of people experiencing homelessness and a range of medical and behavioral health issues they face as they move into and maintain permanent supportive housing. Services will be provided to support quality care in compliance with local, state, and federal regulations and accreditation standards.

ESSENTIAL FUNCTIONS:

1) Act as primary contact for high acuity clients throughout their full length of participation in the ICMS program;

2) Complete a comprehensive intake and Bio-Psychosocial assessment of new participants to help determine needs, eligibility, interests, and goals;

3) Assess and implement necessary ICMS interventions to reduce barriers to housing;

4) Link clients to Medical Home and supportive services such as mental health, health, substance abuse, employment, and education both at the Los Angeles LGBT Center and with external service providers;

5) Provide linkage to resources such as DPSS, Social Security, Medi-Cal, and act as liaison for clients with those resources;

6) Match clients with appropriate housing opportunities through ICMS field-based housing search, including but not limited to short term crisis housing, federally assisted housing, substance abuse programs, and low-income housing;

7) Provide assistance with the completion of all applications for housing placements;

8) Provide linkage to resources assisting clients in completing applications for financial assistance programs;

9) Work with clients and their current and previous supports to develop quarterly ICMS individual care plans to help them meet initial and intermediate goals in the areas of education and employment, housing stability, financial stability, positive healthy outcomes, and permanent connections;

10) Complete quarterly 5X5 assessments including transitions of care documentation following medical appointments and in-patient hospital care;

11) Collect eligibility documents as required;

12) Conduct ICMS required home visits to ensure clients are successfully maintaining their permanent housing;

13) Work effectively and creatively as a member of an interdisciplinary team to support the needs of high acuity ICMS clients;

14) Attend and present clients in weekly case conference meetings to coordinate services with internal and outside service providers;

15) Maintain statistical client data, including entering client data into HMIS Clarity and CHAMP databases as determined by contract requirements;

16) Maintain complete and accurate files documenting all care and services provided in the client’s Electronic Record;

17) Connect with participants and landlords when they are available, including evenings and weekends;

18) Interact in a professional manner with staff and clients;

19) Exercise appropriate judgment and decision-making skills;

20) Use standard office machinery including personal computers and printers in performance of duties;

21) Assist in data auditing tasks, prepare reports as required, and submit in a timely manner;

22) Participate in staff meetings, agency meetings, planning meetings, and other meetings as needed;

23) Comply with requirements of OSHA, HIPAA, and other applicable regulations;

24) Other duties as assigned.

JOB QUALIFICATIONS AND EXPERIENCE:

1) Minimum three years experience providing direct mental health or intensive case management services with people experiencing homelessness;

2) Bachelor’s degree required, preferably in social work/mental health related field;

3) Knowledge of homeless populations and related systems, programs, and Federal policies and guidelines under the Coordinated Entry System (CES);

4) Valid driver’s license with access to reliable transportation and automotive insurance;

5) Training in Motivational Interviewing and Trauma Informed care a plus;

6) Ability to have a “whatever it takes” attitude when working with challenging individuals;

7) Excellent verbal and written communication skills, including strong organizational, detail, and interpersonal skills;

8) Excellent Computer skills and knowledge of Microsoft Office, word processing, Electronic Health Records, and other software systems;

9) Ability to be self-motivated and dedicated to initiating and completing complex tasks;

10) Ability to handle multiple tasks, be detailed oriented, and maintain strict confidentiality of medical information;

11) Able to work well independently with minimum supervision;

12) Bilingual (English/Spanish) skills a plus;

13) Knowledge of or experience working with the LGBTQ community and familiarity with issues of relevance to LGBTQ people;

14) A passion for the Center’s work and its mission to make the world a better place for LGBTQ people;

15) Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
