Our programs are designed around the 7 Dimensions of Wellness, and we provide an array of long-term recovery support services and aftercare to help clients maintain their sobriety.


Role Description

The addiction case manager is responsible for providing support and guidance to individuals in our care for substance abuse and mental health disorders. They will work closely with clients to develop and implement individualized treatment plans, connect them with appropriate resources, and monitor their progress towards recovery.


Key Responsibilities:

●        Work with patients individually facilitating coaching sessions as well as performing case management functions.

●        Have patients complete discharge planning workbooks and assist in all the different areas of need including schedules in place for step down or discharge.

●        Supports the Admissions function by providing tours of the facility, greeting guests and families of patients, and ensuring their needs are met upon arrival.

●        Supports the Clinical and Nursing function by completing general case management tasks/needs. Communicate with the direct care team about needs, concerns, and overall progress of the patients.

●        Assist patients with the step-down process and connecting them with testing applications for ongoing monitoring.

●        Use Teams to communicate effectively with all departments

●        Able to react to change productively.

●        Handle other essential tasks as assigned.


Qualifications:

  • A bachelor's degree is preferred 
  • Peer Support or Recovery Coach certification
  • Strong interpersonal and communication skills.
  • Empathy, patience, and the ability to build rapport with clients.
  • Knowledge of addiction treatment modalities, counseling techniques, and relevant laws and regulations.
