Aliados Health supports our members in four areas: Policy, Population Health, Informatics, and Access and Equity.Position Summary: The Program Coordinator II promotes outreach, education, enrollment, and retention services for various healthcare and nutrition programs, including CalFresh, Covered California, Medi-Cal, Path to Health, Medicare, and other no-cost/low-cost health coverage programs. The position will support community health centers in Contra Costa, Marin, Napa, Solano, Sonoma, and Yolo counties to increase outreach & enrollment capacity by promoting all health insurance and nutrition assistance programs throughout the region. This position is a member of our Outreach & Enrollment team and supports programs and strategies to increase health insurance coverage across our region. Day-to-day activities can vary from calendar and event management to project implementation and one-time assignments. Flexibility, curiosity, and initiative are essential traits for a successful candidate.Essential Duties:Community Outreach & Public Education Promote benefit program awareness and access to healthcare benefit programs at small businesses, schools, faith-based groups, health fairs, community events, and community-based organizations.
  • Assist in targeted outreach to children, families, seniors, the immigrant community, people who are unhoused, and people who do not have health insurance.
  • Conduct community education presentations regarding health insurance, nutrition assistance, and related benefit programs.
  • Represent Aliados Health at community meetings and coalitions to promote inter-agency collaboration for health and nutrition benefit programs and immigrant populations.
Enrollment Support Services
  • Assist households with:
    • Prescreening for health insurance and nutrition assistance benefits
    • Assist clients with obtaining enrollment appointments
    • Obtain clients' signed authorizations and consent to retain information
    • Use Federal Poverty Guidelines to identify patients who are potentially eligible for healthcare insurance and nutrition assistance programs
    • Provide clients with health insurance transition and retention information
  • Contact families when their benefit renewal or recertification is due to offer assistance with the renewal or recertification process.
  • Provide interpretation and facilitate accurate communication for non-English speaking clients.
  • Provide education and information to families about using health coverage, plan benefits, maintaining coverage, and how to avoid coverage interruptions.
  • Attend monthly Aliados Health and program training meetings and other periodic training as assigned.
· Promote awareness and access to healthcare through no-cost/low-cost health coverage programs and CalFresh benefits for small businesses, schools, churches, faith groups, and community-based organizations, including food banks.
  • Provide outreach assistance at assigned health centers or other community partner sites.
  • Perform fieldwork and travel to community events to reach consumers and interact with all levels of the community in a culturally and linguistically appropriate manner.
  • Strictly adhere to State and Federal regulations and policies regarding marketing and enrollment practices for Covered California and Medi-Cal managed care products.
  • Strictly adhere to Covered California conflict of interest regulations governing Covered California’s Certified Enrollment Counselors.
  • Submit timely reports on outreach and enrollment services to the supervisor or department director.
  • Performs other tasks as assigned.
Training & Technical Assistance & Administrative Duties
  • Maintain benefit enrollment expertise in CalFresh, CMSP Programs, Medi-Cal, Medicare, and Covered California.
  • Meet weekly with Aliados Health’s Supervisor to coordinate activities and events.
  • Attend Covered California and Medi-Cal Navigator Program CEC training.
  • Attend CalFresh program training.
  • Participate in health center CEC weekly meetings upon request.
Management Duties (under the direction of the Senior Director of Programs or Department Director):· Plans and coordinates outreach activities and community education with health center staff and staff from community-based organization partners to avoid duplication of services.· Assists in developing and maintaining community partnership relations in assigned geographical areas.· Co-develops and implements strategic community education to promote health literacy for benefit programs.· Serves as a resource to Aliados Health staff for one or more of the following: serving immigrant communities, public charge, working with migrant farmworkers, serving clients with disabilities, and health literacy.· Leads and manages one or more Aliados Health Outreach and Enrollment Peer Groups.· Assists the Department Director with strategic community engagement and education planning, including department webpage updates and social media platforms.Additional Duties:

Knowledge, Skills & ExperienceEducation/Experience· A minimum of a high school diploma or GED is required.
  • Minimum one year experience in healthcare, public health, or related areas.
  • Minimum one year’s experience in administrative/customer service-based work.

Special skills/equipment
  • Owning a car in good working condition and maintaining a valid California driver’s license is required due to necessary work-related travel within Northern California.
