Manager of Employment Transition Services

The Employment Transition Services Manager prepares and delivers a coordinated set of activities that is focused on improving functional achievement of activities including vocational education, integrated employment, adult services, independent living, and community participation. Establishes and implements a plan that is based on the individual’s needs, taking into account the individuals strengths, preferences, and interests. The Manager is responsible for managing the workflow of their team by assigning tasks, supporting staff, monitoring results and reporting to senior management. Contribute to the efficiency of their department by reviewing and improving processes and setting targets for their teams.
ESSENTIAL FUNCTIONS:The Manager will lead a team of Transition Coaches by
Handling leave requests and arranging coverage when necessary
Planning workloads, allocating tasks and monitoring progress against targets
Briefing the team on new initiatives, changes to targets, and company news
Maintaining safety standards, conducting risk assessments, ensuring staff have adequate first aid and fire safety training
Recruiting, training, and helping staff reach their professional development goals
Devising work schedules and implementing deadlines
Ensuring that quality standards are upheld
Conducting performance reviews and disciplinary action as necessary
Participate in the Person-Centered Planning process and review IEP and Individual Service Plans (ISPs) and referral documentation from DDS or school district to ensure that individual preferences are met when working on the job site. Take action to make sure that the strategies discussed in planning meetings are implemented.
Provide direct individualized employment services in accordance with their IP or IEP, UCP policy and procedure. Manage IEPs for individuals within the program.
Implement and make suggestions for curriculum, and lesson plans, and assess and document individual progress.
Develop community based opportunities for individuals to practice skills and develop employment competence. Pair targeted business needs with an individual’s transferable skills.
Develop and foster relationships with school district staff members and case workers to recruit individuals into the program.
Daily and Weekly Communication of information to staff, individuals, and parents regarding individual needs, progress, and resources.
Provide transportation to and from home, program and community based programs. Obtain and maintain the proper passenger endorsement on your driver’s license that will allow you to transport individuals to and from work/community site/home.
Perform assessments to identify each individual's strengths, interests, and preferences.
Conduct work evaluations by observing and documenting each individual's ability to engage in competitive employment, identify needs for accommodations, assess skills and abilities.
Use structured intervention techniques to assist the individuals to perform job tasks to the employer’s specifications.
Provide work support on job sites helping the individuals to maintain and be successful in their employment.
Provide training and support to individuals to promote independence at their work sites.
Provide instruction for teaching individuals to work tasks and appropriate social skills.
Identify the natural supports occurring in the workplace to assist employees with disabilities to perform their jobs. Facilitate the transition to natural workplace support.
Encourage individuals to maximize productivity and enhance relationships with supervisors and co-workers.
Establish and maintain relationships with employers at work sites.
Assist with personal care and toileting needs of program participants as needed.
Maintain regular communication with employers to review productivity and quality of work.
Ensure individual safety by familiarizing individuals with safety rules and regulations at their work sites.
Keep timely and accurate progress notes and comprehensive reports.
Ensure compliance with all company policies and procedures.
Maintain all individual documentation/files and maintain HIPAA compliance for privacy while ensuring best practices. Under the HIPAA Privacy Rule, this position has access to individual Protected Health Information (PHI).
Participate in staff meetings, in-service training, staff development programs, professional meetings, conferences, and related programs as requested.
REQUIREMENTS, SKILLS, ABILITIES & KNOWLEDGE:
● Skill in working with and teaching individuals with disabilities.
● Ability to pass and maintain professional certifications, including CESP.
● Ability to communicate effectively in a professional manner.
● Ability to stoop, kneel and lift to complete assigned tasks
EDUCATION & EXPERIENCE:
High School Diploma (required)
Bachelor’s Degree in an applicable field (required)
Supervisor Experience (required)
One to two years related experience (desired)
