JOB TITLE: Case Manager, Troxel Center

STATUS: Full-time/Non-Exempt

REPORTS TO: Director of Women & Children SCHEDULE: Varies, Monday-Friday

SUMMARY OF POSITION:

Be spiritually sound and mature Christian that is able to firmly and graciously lead single women and women with children towards life change according to program guidelines. To work with residents to develop an individual care plans that incorporates the Four Pathways to Progress (Spiritual Growth, Community, Industry, and Education) and counsel to facilitate growth and development towards a healthy, addiction-free life. Coaching should be Biblically based, pointing to our mission, vision and values, addressing the needs of the whole person.


DUTIES AND RESPONSIBILITIES

 Identify and assess resident needs and goals and develop a written individual care plan to meet those needs and goals. Work with the team to ensure care plan is relevant and comprehensive.

 Assist with interviewing, intake and move in process as needed.

 Meet with residents weekly and provide Biblically based coaching to stimulate growth, help guide in making decisions, teach problem solving skills, encourage the turning away from destructive behaviors, navigating change, and increase the resident’s competence and autonomy.

 Meet with departmental staff to discuss resident progress in order to provide the most excellent and comprehensive service available

 Document in Mission Tracker and maintain current and complete case notes and records for each resident in compliance with program standards and requirements

 Complete monthly and year-end program statistical information and submit by deadline for compilation of reports.

 Teach classes and facilitate groups as needed

 Collaborate with resident to schedule resident’s internal and external meetings and appointments, insuring the appropriateness of services.

 De-escalate situations that have the potential or likelihood of becoming volatile or uncontrolled; take action according to standards and protocol.

 Transport to and from appointments, court hearings, doctors, community agencies, Walmart, etc .

 Assist with administrative program functions such as gratuity dispersal, maintaining resident roster, and ensuring organized, complete, and current resident files.

 Conduct daily room and chore checks.

 Assist with supervision of work assignments at the facility.

 Perform urinalysis for residents.

 Attend staff meetings and perform other duties as requested.



SKILLS AND QUALIFICATIONS

 Ministry minded with the passionate belief and commitment to invest in individuals, demonstrating Christ’s love and grace through relationship healing and practical assistance

 Grounded in the Christian faith as a mature, active member of the body of Christ

 Excellent team player, willing and able to collaborate with and support staff members

 Strong interpersonal skills, able to listen and guide/counsel according to Biblical principles and individual client needs

 Must have and model healthy emotional and physical boundaries and personal safety

 Strong writing and verbal skills

 Ability to remain calm and reassuring when dealing with upset clients

 Ability to access and procure needed information and resources; good negotiation and networking skills

 Ability to handle multiple tasks simultaneously while achieving program outcomes and meeting deadlines

 Adaptable, flexible, honest and perceptive

 Organization with regard to people, things, and information, including excellent time management skills

 Computer literate - proficient in word, excel, and internet

 Experience in addition, recovery, homelessness, and mental health services

 Valid Florida driver’s license, record acceptable to LMI insurance

Minimum Bachelor’s degree in social work, counseling, human services or education field
