Job Overview: We are seeking a dedicated and compassionate Dialysis Patient Care Coordinator to join our healthcare team, specializing in coordinating care for dialysis patients. The ideal candidate will be responsible for managing and facilitating appointments, interactions, and procedures related to dialysis treatment, working closely with dialysis units, nephrologists, hospitals, and our vascular surgeon and interventional radiologists. This role requires a strong background in patient scheduling within a medical setting and a genuine commitment to providing high-quality patient care.

Responsibilities:

  1. Dialysis Patient Coordination:
    • Schedule and coordinate appointments for dialysis patients.

Liaise with dialysis units to ensure seamless patient transitions and timely treatments.

Act as a central point of contact for dialysis patients, addressing their queries and concerns.

  1. Communication with Healthcare Providers:
    • Collaborate with nephrologists to ensure alignment of care plans and scheduling requirements.

Communicate effectively with hospitals to coordinate dialysis-related procedures and treatments.

Facilitate clear communication between the patient and healthcare providers.

  1. Scheduling Hospital Procedures for Vascular Surgeon:
    • Manage and schedule hospital procedures for our vascular surgeon.

Coordinate pre-operative and post-operative care for patients undergoing vascular procedures.

Ensure efficient use of resources and timely completion of procedures.

  1. Patient Advocacy and Support:
    • Demonstrate empathy and understanding in interactions with dialysis patients.

Advocate for patient needs and preferences during the scheduling and coordination process.

Provide support to patients in navigating the healthcare system.

Qualifications:

  • Previous experience in patient scheduling within a medical environment is essential.

Knowledge of dialysis procedures, terminology, and related healthcare practices.

Strong organizational skills with the ability to manage multiple tasks efficiently.

Excellent communication and interpersonal skills.

Compassionate and patient-centered approach to healthcare coordination.

Proficiency in relevant software and scheduling tools.

Education and Experience:

  • Bachelor's degree in healthcare administration, nursing, or a related field is preferred.

Previous experience in a patient care coordination role is highly desirable.
