Hospice RN - Team Lead

Care Partners is seeking a Registered Nurse for the position of Hospice Nurse – Team Lead to provide coverage in the Portland metro area. This position has candidates in the field providing direct patient care. We are a trusted name amongst our community partners and receive consistent feedback from families that our nurses go above and beyond expectations to make them feel supported at their loved one’s end of life.

Your Responsibilities:

  • Act in a lead role, providing clinical direction, advice and assistance to day shift and evening shift nurses to assure that patients receive high quality palliative and hospice care.
  • Facilitate communication between nursing personnel on two shifts to assure that crucial patient care needs are met in a quality, effective manner.
  • Provide telephone advice to patients, families, and caregivers, problem-solve patient visit schedules, and coordinate care between daytime and night-time nursing teams.
  • Coordinates patients visit schedules to assure patients receive timely appropriate care
  • May provide nursing advice and triage to patients, families, and caregivers via telephone when their RN case manager is unavailable.
  • Provides skilled nursing assessment, planning and coordination, care management across settings to patients in a private home, adult foster home, residential/assisted living care, and/or nursing facility.
  • Assesses patient/family needs and consults with attending physician, Interdisciplinary Group (IDG) staff members or other appropriate community contacts as indicated at the time of the assessment.
  • Serves as a clinical and management resource for patient care problem solving. Acts as liaison between staff, physicians, and management.
  • May provide all professional nursing services in accordance with the orders of the attending physician, and within the RN scope of practice.
  • Manages medications in the patient’s home/residence upon the specific orders of the physician.
  • Consults with Medical Director, Primary Physicians, medical specialists, and pharmacists for recommendations of medications and/or treatment considerations.
  • Delegates and assigns tasks to the Hospice Aides (HA) or non-family members and caregivers.
  • Supervises the care provided for HA services and/or a Licensed Practical Nurse (LPN) as required by law.
  • Assures professional competency through maintaining skills and knowledge consistent with state and Medicare regulations and Oregon Nurse Practice Act and all organization policies, work rules and standards.
  • Maximizes the comfort and health of patients and families consistent with hospice philosophy and the organization’s policies and procedures.
  • Provides phone triage and support to patients/families as needed and requested.
  • Maintains up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur in accordance with policies and procedures.
  • Participates in the development of the plan of care for patient/family and Interdisciplinary Group (IDG) input, including preventive and rehabilitative nursing procedures appropriate for the patient’s care and safety.
  • May attend case conferences and actively participate in discussion of patient needs and care planning.

Qualifications:

  • Requires current unencumbered Oregon licensure as a Registered Nurse prior to first day of employment.
  • Must have at least 2-4 years of hospice experience and 1-2 years of hospice clinical leadership experience
  • Prior experience coaching, guiding and/or on the job training to others is highly desirable, including flexibility to be successful with a variety of work styles.
  • Must have a valid driver’s license and acceptable driving record and have access to a reliable automobile to travel to and from patient homes.
  • Requires well-developed critical thinking, patient assessment, and problem-solving skills.
  • Requires the ability to work independently and as part of an interdisciplinary team with strong verbal and written communication skills.
  • Must have good skills in using electronic tools such as email, internet, keyboarding and be able to learn medical software and job-related electronic applications to use for accurate and timely charting and administrative purposes.
  • Bi-lingual, Spanish speaking ability and certification in Hospice and Palliative Care are helpful.
  • Basic life support CPR card or ability to obtain a valid card prior to patient contact within two weeks of hire is required.

Schedule: The schedule includes 10-hour day shifts three days a week: Friday, Saturday, and Sunday. Work hours are from 7am-5:30pm

*Preferred applicant will be asked to pass a background check and drug screen following a job offer. **New employees are required to complete tuberculosis screening and provide proof of hepatitis B vaccination or signed declination.
