Hospice Nurse

Company Overview: Gentiva Hospice, a leading provider of hospice and palliative care, is seeking a Registered Nurse to join their team in Portland, ME.
Work Schedule: This full-time position offers a 7 shifts on, 7 shifts off schedule, providing a healthy work-life balance.
Job Responsibilities:
- Provide high-quality patient care services to hospice patients and support their families after normal business hours on an on-call basis
- Lead an interdisciplinary team to enhance the quality of hospice care
- Evaluate patient/family needs and assist in the development and implementation of individualized care plans
- Maintain accurate and up-to-date patient clinical records
- Participate in company-wide quality assessment and performance improvement initiatives
Requirements:
- Current RN license
- 1+ years of nursing experience in hospice, home health, or oncology preferred
