Work Schedule: This full-time position offers a 7 shifts on, 7 shifts off schedule, providing a healthy work-life balance.

Job Responsibilities:

  • Provide high-quality patient care services to hospice patients and support their families after normal business hours on an on-call basis
  • Lead an interdisciplinary team to enhance the quality of hospice care
  • Evaluate patient/family needs and assist in the development and implementation of individualized care plans
  • Maintain accurate and up-to-date patient clinical records
  • Participate in company-wide quality assessment and performance improvement initiatives

Requirements:

  • Current RN license
  • 1+ years of nursing experience in hospice, home health, or oncology preferred
