Plans of Safe Care Liaison Description:

The Plans of Safe Care Liaison is to partner with Multi-disciplinary Teams within Erie County.

Plans of Safe Care (POSC) programs are designed to ensure the safety and well-being of infants affected by prenatal substance exposure and to support their caregivers. These Plans are part of the Child Abuse and Prevention and Treatment s ACT (CAPTA) and aim to address both health and substance use disorder treatments needs for infants and their families. 

Here are some key aspects of the program:

  1. Comprehensive Approach: POSC involves a collaborative, cross-system approach that includes healthcare providers, child welfare services, and substance use treatment programs.
  2. Individualized Family Plans: Each plan is tailored to the specific needs of the infant and their family, ensuring that both receive the necessary support and services.
  3. Support for Caregivers: The plans also focus on the recovery and well-being of the caregivers, recognizing that a stable and healthy caregiver is crucial for the infant’s development.

 Qualifications:

  • Bachelor’s degree in Psychology or Social Work, preferred or Bachelor’s in related field with a Psychology minor required.
  • 1-3 years of work experience with community referrals, supporting, collaborating and goal planning with children and families.
  • 2-5 years of work experience and knowledge of the recovery/relapse process and community and court supports in the areas of addiction and recovery.
  • Experience working in a medical/hospital, treatment program environment.

  

Additional Requirements: The candidate must pass a Criminal Record History Clearance, FBI and Child Abuse History Clearance, possess a valid PA Driver's License and must pass a physical exam and drug screening. Candidates must also complete the most current Mandated Reporter training.

Work Expectations:

  • Encounter and assess the client and their immediate family needs. 
  • POSC staff and client will work together to identify smart, realistic, obtainable and time sensitive goals to develop some stability for the client and family. 
  • Complete various family specific assessments to determine outcomes for family plan.
  • Participate and prepare for Multi-disciplinary Teams within Erie
  • POSC coordinator will support through case management services, open weekly group sessions and will assist the family with obtaining resources in the community for the client or the children. 
  • The POSC coordinator will offer support and assistance with referrals to outside agencies and will complete the referrals when a need is identified.
  •  The POSC coordinator will maintain regular weekly contact with the family to make sure they are working on their goals and the family needs are being met.

 

Plans of Safe Care Coordinator will provide:

Direct case management:

  • In-person/virtual visits/travel for in-person visits
  • Initial meeting
  • Family 30-, 60- and 90-day visits. In-person visits may continue until the child’s first birthday if determined to be necessary in the POSC.
  •  The capacity to work supportively, collaboratively, and effectively in a team environment.
  • Ability to take initiative independently with various tasks and exhibit proven project management skills.
  • Strong observation, communication and writing skills to clearly convey information in writing and in person. (Ex.: Microsoft Office skills, Excel)
  • The ability to interact with a diverse population of individuals and families professionally and positively.
  • Exceptional organizational and time management skills with the ability to work accurately and efficiently in a fast-paced environment with multiple staff and clients at once.
  • High commitment to respect and confidentiality of family information and case details with adhering to confidential ethics among those who are on a need-to-know basis.
  • High commitment with respect to cultural responsiveness and providing a culturally sensitive atmosphere for staff, families, and community.
  • Remain current and seek out the latest trends in family addiction and recovery.
  • Tolerance for working in an active environment with many adults and children present.
  • Able to adapt to physical requirements to sit and/or stand for prolonged periods, occasionally requires the ability to stoop, bend, lift and reach.
  • Program may require travel in and out of Erie County and Pennsylvania.

 

Additional expectations:

  • Create and advise on Child Protective Services safety plans, substance use treatment plans and hospital discharge plans.
  • Partnering with birthing hospitals, substance use disorder treatment providers, primary care providers, home visiting professionals, public health agencies and other community partners as needed.
  • System collaboration and coordination are essential in the success of the development, implementation, and overall effectiveness of each plan with a Multi-disciplinary team (MDT).
  • Identification of parent barriers to accessing services: transportation, financial and/or language along with strategies to overcome these barriers.
  • Stigma reducing practices designed to engage the patient in consistent prenatal care;
  • Referrals to Family Strengthening, Early Head Start, Family Check Up for Children, Healthy Families America, Nurse-Family Partnership, Parents as Teachers, Family Group Decision Making (FGDM), Women Infant Children (WIC), public assistance, transportation assistance, counseling, housing assistance, domestic violence programs and/or food banks, etc.
  • Referral to ChildLine if there are concerns with parent/s ability to be a caretaker for other children.
  • Modifications to a Plan of Safe Care are completed at any time an increase or decrease in intervention is determined to be appropriate.
  • Careful assessment of a mother’s substance use in determining how to implement a Plan of Safe Care.
  • Engagement of family supports, identification of family strengths, understanding and adhering to the highest form of confidentiality and the importance of signing of releases for mother’s and caregiver’s providers.
  • Provide group session using the Families in Recovery 7-week curriculum.
  • Provide any necessary support as deemed appropriate by the Program Supervisor or Chief Operating Officer.
