About the Role

We are seeking a highly skilled Care Coordinator to join our team at UnitedHealth Group. As a Care Coordinator, you will play a critical role in ensuring that individuals receiving Long-Term Services and Supports (LTSS) receive high-quality, person-centered care in a supportive and efficient manner.

Key Responsibilities
  • Assess and develop individualized care plans that address the unique needs and goals of each person
  • Coordinate with healthcare providers, community resources, and family members to ensure seamless care transitions
  • Identify and address social determinants of health that impact an individual's well-being
  • Communicate effectively with stakeholders to ensure timely and coordinated care
  • Advocate for individuals and families to ensure their needs and preferences are respected and supported
Requirements
  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or related field
  • 2+ years of experience working in a community health setting, preferably in a healthcare role
  • 1+ year of experience working with individuals with long-term care needs
  • 1+ year of experience working with individuals receiving services on a home and community-based waiver in Kansas
  • Proficiency in MS Word, Excel, and Outlook
Preferred Qualifications
  • Licensed Social Worker or clinical degree
  • Background in managing populations with complex medical or behavioral needs
  • Experience with electronic charting
  • Experience with arranging community resources
