Patient Care Coordinator Opportunity

We are seeking a dedicated and compassionate Patient Care Coordinator to join our team at Patriot Home Care. As a Patient Care Coordinator, you will play a vital role in ensuring the highest quality of care for our patients in the comfort of their own homes.

Key Responsibilities:

  • Screen new patients and caregivers to ensure the best possible match
  • Onboard new patients and caregivers, providing assistance with documentation and clearance requirements
  • Staff patient cases with caregivers and ensure timely start-ups
  • Communicate with patients, caregivers, and families to ensure satisfaction and quality service delivery

Requirements:
