POSITION SUMMARY: The Licensed Practice Nurse II (LPN) is responsible for rendering nursing care in an assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the registered nurse. This position has responsibility for demonstrating the core values of CareSTL Health: Customer Service, Community, Commitment, Compassion, and Competence.


ESSENTIAL FUNCTIONS:

The following information is considered the definition of essential functions but does not restrict the tasks that may be assigned. The Licensed Practical Nurse II (LPN II) may be reassigned duties and responsibilities at any time due to reasonable accommodation or other staffing reasons. Other duties may be assigned.


  • Communicate with physicians and develop a working relationship and evaluate the patient to determine the level of care, when the patient needs further, home planning and other needs and action plans and review treatment goals and resources
  • Prepares and administers medications and treatments with knowledge of the purpose and adverse effects of nursing care provided as well as prepares equipment and aids Physicians during treatment and examination of the patient while ensuring that patient tests are appropriate and necessary and maintaining appropriate costs
  • Assists with providing patient care: records vital signs; discusses and records chief health concerns; performs tests; administers injections as well as administers prescribed medications and treatments in accordance with the scope of practice
  • Completes expanded assessment of patient at the time of initial/new clinic encounter, interprets medical documents and facilitates investigations, medical and treatment referrals, and participates in the development and implementation of patient care policies and protocols
  • Helps patients understand their conditions and treatment options while providing education and literature about diagnosis while helping patients and families make informed decisions as well as explains procedures and treatments to patients to gain cooperation, understanding, and alleviate apprehension



OTHER FUNCTIONS:

  • Places phone calls to pharmacies to order prescriptions; sets up referral contacts
  • Refers to cases where patients and/or family would benefit from counseling and coordinates the integration of social services/case management function
  • Evaluates patient satisfaction and quality of care provided and maintains awareness of comfort and safety needs of patient
  • Promotes effective and efficient utilization of clinical resources

JOB REQUIREMENTS AND QUALIFICATIONS:

Education:

  • Graduate of an accredited school of Nursing is required
  • Valid registration as a Licensed Practical Nurse in the State of Missouri required
  • Current AHA Healthcare Provider training (BCLS) required

Experience:

  • 6+years of experience in a community health center and/or hospital setting

Knowledge, Skills, and Abilities:

  • Knowledge of information and techniques for providing excellent patient care
  • Knowledge of administrative and office procedures and systems such as electronic transmission via facsimile and/or internet
  • Good computer skills including the use of office procedures and terminology
  • Good self-motivational and organizational skills
  • Ability to maintain confidentiality
  • Ability to work all health center sites
  • Understanding of the complexities of integrating services
  • Knowledge of data management through the use of IT

Transportation Requirement:

  • Must have reliable transportation

Supervisory Responsibility:

  • None

Additional Eligibility Qualifications (licenses, programs, certificates, pre-employment check and post and annual screening(s).

  • Copy of High School Diploma/GED as applicable or Official Transcripts
  • Copy of Missouri license as Practical Nurse
  • Current AHA Healthcare Provider training (BCLS)
  • Current Registration with the Missouri Family Care Safety Registry
  • Annual PPD or Chest X-Ray

Additional Information:

Clinic hours are Monday - Friday 8 a.m. to 5 p.m.
11 paid holidays per year.
