The Developmental Disabilities Department is looking for someone who is passionate about helping and serving our community. This individual will be providing services in home, community, and healthcare settings for individuals who experience Intellectual and/or Developmental Disabilities.

Position: Service Coordinator

Department: Developmental Disabilities

Shift: Full time (Monday – Friday, 8:00am to 5:00pm)

Salary Range: Annual salary range is $56, 347.20 to $72, 238.40. We use a bona fide factor system to determine starting wage (years of experience, education, credentials, and specialty skills that relate to the job requirements).

Position Requirements: High school diploma and three years in human services with at least one year working with I/DD clients required. Case management experience in Oregon preferred. Combination of education/experience that demonstrates the ability to perform the duties outlined for this position may be considered.

Summary: CCMH empowers individuals with intellectual and developmental disabilities, mental health challenges and other social barriers to lead self-determined lives and reach their full potential As a Service Coordinator, you'll be responsible for implementing and monitoring systems that support the IDD community. You'll facilitate health/safety, enhance people's quality of life, help them lead their desired lifestyles and gain increased levels of independence, productivity, and social/physical integration.

Working conditions: This position works in an office environment as well as in the community. This position requires driving to different locations within Columbia County and some occasional driving to locations elsewhere in the Portland Metro. The Service Coordinator will also be available to provide services in the client's residence as needed.
