Job Title:  Prolonged Field Care Training

Location: NC, USA

Duties:

  • The goal of this contract is to routinely provide medical training to medical and non-medical personnel on small Advising Teams (ATs) to independently manage casualities and provide prolonged field care for up to 72 hours from the point of injury/illness through evacuation to the next echelon of care. SFAB medics and non-medical team members must conduct synchronized medical proficiency training in prolonged field care through the conflict continuum in order to enhance mission capabilities and increase SFAB readiness for evolving operational environments. The unit requires means to effectively train medical and non-medical personnel for prolonged field care scenarios in austere environments with no to minimal external medical support
  • Provide a five-day team-centric medical training course focusing on prolonged field care (PFC) and austere intervention covering PFC critical tasks for up to twenty (20) medics and non-medics, ten (10) courses annually.
  • Train an Advsior Team (AT) to triage, evaluate, and manage a casuality during prolonged care without robust medical supervision or support.
  • Train Tactical Combat Casualty Care (TCCC) trained non-medical personnel and 68W trained medics/medical personnel in parallel in order to execute a culmination exercise as a team with both types of personnel with an established and standardized course syllabus designed by the contractor to train all aspects of PFC to assist in the prolonged treatment of a casualty.
  • Train proficiency in the ten core PFC capabilities [monitor, resuscitate, ventilate & oxygenate, airway management, sedation & pain control, physical exam & diagnostics, ongoing care, advanced procedures, telemedicine, and prepare for evacuation] aligned with the most up to date recommendations outlined in the Joint Trauma Systems PFC clinical practice guidelines.
  • Provide students the ability to perform life-saving surgical procedures, such as sternal IO, surgical cricothyrotomy, chest tube, Foley catheterization, intravenous cannulation on amputee role-players wearing wearable and realistic medical procedure devices.
  • Increase the capability and expand the scope of surgical and critical care medical practice in order to mitigate urgent life, limb, or eyesight emergencies.
  • Utilize and train on medical equipment that mimics organic unit capabilities including but not limited to portable ventilators, EMMA end tidal capnography, portable monitor, man packable ultrasound units, advanced airway procedures manikins, infrascanners, field microscopes, blood temperature monitoring devices, golden hour boxes, and high fidelity manikins.
  • Practice using 3rd world, unconventional, improvised, and primitive medicine.
  • Provide training consisting of three days of classroom training (lectures and hands on practicals), a day for cadaveric tissue lab, and a 12-18 hour culmination scenario focused on PFC for both medics and non-medics.
  • The cadaveric tissue lab encompasses orientation to relevant anatomy, immediate life life-threatening conditions, ventilation management, thoracic interventions and management, vascular access, field ultrasound, suturing/closures, long term wound care, and austere surgical interventions. 1.3.13. Provide the personnel, management, materials, equipment, and facilities necessary to educate, familiarize, and provide hands-on training for care of a patient in excess of 24 hours. 1.3.14. Integrate vehicles of opportunity in the final medical scenarios. 1.3.15. Satisfy the 40 hour Non-Trauma Training requirement for the National Registry Emergency Medical Technician refresher

 

Education & Qualifications:

 

  • Medical Director: Medical Director must be a licensed and practicing Emergency Medicine or Critical Care Medicine physician. Medical Director must have a minimum of five (5) years experience as a Special Operations/SFAB Provider/Surgeon. Previous experience as a member of the Prolonged Field Care Working Group considered as HIGHLY QUALIFIED. Instructors must have satisfactory, documented, and verifiable experience in the delivery of PFC training to SOF/SFAB personnel with the past two (2) years.
  • Medic Primary Instructor: Licensed Physician or Physician Assistant with minimum four (4) years experience in a Special Operations/SFAB unit as a Medical Provider. Must have a minimum five (5) years SOF/SFAB medical experience as an MOS 18D, SARC, or SOIDC operator or leader in either a Special Mission Unit (SMU), U.S. Army Special Forces, Navy SEALs, Marine Special Operations Command, SFAB, or U. S. Army 75th Ranger Regiment to include award and retention of their Military Occupational Specialty Qualification (MOS Q). Instructors must have satisfactory, documented, and verifiable experience in the delivery of PFC training to SOF/SFAB personnel with the past two (2) years. Previous experience as a member of the Prolonged Field Care Working Group considered as HIGHLY QUALIFIED.
  • Non-Medical Primary Instructor. Instructors will have no less than six (6) years progressive experience in U.S. military special operations unit / SFAB and have attended and graduated from the Special Operations Combat Medic Course. Instructors will possess valid and current National Registry EMT and National Registry Paramedic. If Licensed RN, consider HIGHLY QUALIFIED for position. Instructors must have satisfactory, documented, and verifiable experience in the delivery of PFC training to SOF/SFAB personnel with the past two (2) years. Previous experience as a member of the Prolonged Field Care Working Group considered as HIGHLY QUALIFIED.
  • Subject Matter Expert – Ultrasound: Instructor will be an advanced Subject Matter Expert in Point of Use Ultrasound in the SOF/SFAB medical realm. This SME shall be capable to provide instruction on eFAST/ FAST exam, peripheral/central catherization and regional blocks. This SME shall be recognized in his field as an expert through certifications/license documentation. Instructor position considered highly qualified if previous experience working within SOF/SFAB Medical Community.
  • Subject Matter Expert – Airway and Ventilator Support: Minimum Instructor requirement of advanced SME who has documented certifications and considered highly preferred as a Registered Respiratory Therapist (RRT), Respiratory Care Practitioner (RCP) for instruction of advanced airway periods of instruction. Instructors is considered highly qualified if having worked in SOF/SFAB environment in last 5-years with documented experience in instruction of advanced subjects pertaining to PFC.
