Job DescriptionJob Description

Position Summary: The Youth & Family Specialist will provide home and community support to promote safety and stability through assessment, direct intervention, care coordination and referral to appropriate services to help meet their needs.

Location: Topeka

Education: Bachelor’s degree in social work, counseling, criminal justice, child and family development, education, or related field. Years of related work experience may be substituted for years of education.

Licensure/Certification: Current Driver’s License and Auto Insurance

Preferred Skills: Two or more years’ experience in children and family services preferred Prevention Services and in-home services experience BSRB Licensure within the state of KS preferred but not required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Conduct Initial Family Assessments: Establish rapport with families, assess their strengths, needs, and challenges using standardized tools, and document findings accurately.
  • Identify and Prioritize Needs: Analyze data to identify key intervention areas and prioritize based on urgency and impact on family stability.
  • Develop Individualized Service Plans: Collaborate with families to create personalized plans using a strengths-based approach, ensuring alignment with program goals.
  • Connect Families to Services: Refer and coordinate access to appropriate services, advocate for timely and culturally responsive support, and regularly review service plan effectiveness.
  • Educational Workshops and Training: Offer sessions on parenting skills, child development, and life management, and strengthen protective factors within families.
  • Documentation and Reporting: Generate required reports, participate in team meetings, and ensure compliance with laws, regulations, and organizational policies.
  • Collaborative Partnerships: Build and maintain relationships with community stakeholders to enhance service delivery and resource accessibility.
