Nurse Wellness Educator-Coral Springs

 

My Health Onsite operates onsite and near-site health and wellness centers. We deliver advanced personalized work-site healthcare solutions to employers that enhance patient engagement while proactively improving healthcare outcomes. Our medical team takes time to build strong relationships. No one is rushed in and out, and no one is a “number.” Patients may access a range of medical services including x-ray, an onsite pharmacy, wellness services, treatment for acute illnesses and chronic conditions. Our programs go beyond caring for the sick and injured – we make prevention our number one goal. 

 

Summary

 

Promote the overall health and wellness of our Coral Springs patient population through face-to-face and telephone interactions. Implement and manage wellness program initiatives, presentations, and events that create awareness and motivation to help patients adapt and maintain a well-rounded healthy lifestyle. 

 

 Schedule: Monday 8a-6p, Tuesday 7a-5p, Wednesday 8a-4p, Thursday 10a-7p, Friday 8a-4p-closed for lunch 12p-1p Tuesday, Wednesday and Friday and 1p-2p Monday and Thursdays

 

Essential Responsibilities

 

  • Develop, coordinate, manage and maintain total well-being programming to reduce health risk, medical costs and encourage healthy lifestyles
  • Educate patients regarding their illness and treatment, including self-care, prevention and health maintenance
  • Help patients develop achievable and measurable wellness goals to monitor success and motivate ongoing behavioral change
  • Case Management of VSR (Vital Systems Review) Program
  • Seek out community resources that would be beneficial to the patient’s wellness goals
  • Create well-defined metrics to measure the success of the programs
  • Stay on top of any changes to health and safety regulations with government agencies
  • Document all assessments, actions, care and treatment responses, and communications in patient record in EMR
  • Maintain patient confidence and ensure confidentiality of patient care information

 

Minimum Qualifications

 

  • Education: Associate’s degree from an accredited college or university
  • Experience: At least 2 years of experience, with strong preference for primary or urgent care setting. Experience managing a wellness program and making FTF presentations
  • Licensure:  Current registration with the Florida State Board of Nursing as a RN or LPN
  • Travel: TBD
  • Certification: BLS for Healthcare Providers
  • Preferred Certifications: Certified Diabetes Educator (CDE), Certified Health Education Specialist (CHES), Tobacco Treatment Specialist (CTTS)

 

Knowledge/Skills/Abilities

 

  • Proficient knowledge of computer systems including electronic medical record (EMR)
  • Understanding of public health issues and employee health and wellbeing trends
  • Ability to effectively communicate with staff and patients using excellent written and verbal skills
  • Friendly personality and ability to work well as a team member
