Minimum Experience: Experience working with individuals experiencing severe and persistent mental illness preferred

The Assertive Community Treatment (ACT) team provides non-traditional community based mental health treatment to promote the recovery and community integration of adults with serious and persistent mental illnesses and co-occurring substance use and/or serious co-occurring physical health conditions. The overall goal of the ACT team is to reduce use of ER’s, inpatient hospital stays, and other harmful behaviors. The ACT Case Manager is an integral part of a multi-disciplinary team of mental health clinicians/providers. This position will provide general case management, education regarding daily living skills and overall wellness, and other matters related to assisting participants to live more independent lives. The ACT case manager will be expected to manage his/her own schedule and will be expected to work in a team environment. The vast majority of services will take place in participant’s homes and/or in the community. Will be required to be on call for a 7 day rotation every 6-8 weeks.

  1. Provide competent case management services in the community as assigned by supervisor.
  2. Provide case management services based on the philosophies and principles of emphasizing participant’s strengths.
  3. Assist in identifying, providing assertive outreach to, and establishing rapport with new case management patients.
  4. Assist in leading psychosocial groups as needed and/or requested by supervisor.
  5. Assess strengths and set goals with the participant and when appropriate with involved others; review regularly.
  6. Be well informed regarding participant’s physical health needs. Engage participants in a variety of ways to increase understanding of health condition/health literacy and motivation to engage in self-management. Encourage participants to monitor and manage their health, emphasizing self-direction and skills development.

Engage in the following patient-centered case management activities as needed by each participant:

  1. Actively coordinate case by providing case consultation services and attending treatment meetings with internal staff and external agencies/organizations as needed.
  2. Employ problem solving with patient and when appropriate with involved others to help them resolve issues and problems.
  3. Provide appropriate crisis intervention assistance.
  4. Assist the participant in getting linked up with appropriate services based upon need.
  5. Encourage participant to become more actively involved in the community.
  6. Provide support and advocacy with various systems on behalf of participant to facilitate obtaining needed resources and/or protection of rights.
