Intensive In-home Therapist Lead

Summary:
Perform duties associated with admission and retention of new and existing consumers to Intensive In-Home Program. Provide clinical expertise and oversight for the Intensive In-Home Team. Coordinates with Director of Community Based Services.
Principal Duties and Responsibilities
Specific duties and responsibilities:
Clinical:
- Coordinate delivery of time-limited individual and intensive family treatment services intended to stabilize the home environment, promote reunification, or prevent the utilization of out-of-home placement (i.e. psychiatric hospital, therapeutic foster care, residential treatment facility) for the identified youth within designated time frames.
- Completes required documentation to enroll and retain consumers in Intensive In-Home Services provided by CHA, including but not limited to: gathering previous treatment histories, development of initial/complete Person-Centered Plan, completion of all paperwork associated with maintaining authorization, and making necessary referrals. Makes appropriate referrals for clients who are referred and do not meet criteria to receive services provided by CHA.
- Ensures agency standardized instrument panel is completed on all clients admitted into the program.
- The team leader shall participate in treatment or oversee the following in relation to treatment:
- Defuse the current crisis, evaluate its nature, and intervene to reduce the likelihood of a recurrence.
- Ensure linkage to needed community services and resources.
- Provide self-help, social and basic living skills training for youth.
- Provide parenting skills training to help the family build skills for coping with the youth’s disorder.
- Monitor and manage the presenting psychiatric and/or addiction symptoms.
- Work with caregivers in the implementation of home-based behavioral supports. Services may include crisis management, intensive case management, individual and/or family therapy, skills training, and other rehabilitative supports to prevent the need for an out-of-home placement or more restrictive services.
- Provide individual and family therapy at a frequency consistent with the client needs.
- Work with natural and community supports (school, doctors, daycare, DSS, DJJ, faith communities, housing, food pantries, etc.
- Employ agency approved evidenced based practices for the delivery of services,
Administrative:
- Provides administrative oversight to the program, including managing program utilization, CQI, client outcomes, and client marketing of the program.
- Provide quality aftercare to clients after discharge.
- Ensures program operations within a budget and spending policy. Ensure program achieves monthly utilization targets and promote strategies that lead to the program exceeding budgeted production.
- In conjunction with the Director of Community Based Services responsible for recruiting, hiring, and training new staff.
- Provide administrative supervision to the Intensive In-Home team members through weekly team meetings and monthly individual supervision.
- Available for crisis intervention and/or supervision 24 hours a day.
- Maintain Licensure and Actively participate in individual supervision for licensure and responsible for keeping personal record of supervision and getting information to the state Licensing board so that Licensure will be obtained within 2 years of date of hire. Complete yearly CEU's as required per licensing board.

Intensive In-Home Team Members
Core Competency: Analytical Skills
- Therapist demonstrates an ability to review, interpret, and include client assessments in case conceptualization of treatment plan. Includes internal and external CCA’s, PCPs, Medical Evaluations, and Psychological Testing.
- How will competency be measured? At hire review of staff member's first client admission for evidence that inclusion of collateral information is included in client assessment, progress notes, and treatment plan. Annual review of 5 client records for evidence that inclusion of collateral information is included in client assessment, progress notes, and treatment plan.
Core Competency: Supervisors
- Supervisor’s will complete 100% of direct report’s evaluations on time (within 30 days of the effective date of evaluation) and will have no old evaluations outstanding to meet expectations. Supervisors that supervise second level reports will also need to have 85% of any indirect reports completed on time to meet expectations.
- Measurement: Review of evaluations due during the review period and whether they were completed on time.

Education and Experience Requirements
- Minimum of Master’s Degree and 1 year experience in a human service field and North Carolina licensure or provisional licensure in one of the following disciplines: LCMHC(A), LCSW(A), LMFT(A); Plus a minimum of 1 year experience working with same or similar client population required.
- Requires knowledge and the ability to practically implement mental health regulations and clinical services.
- Requires effective communication, decision making, analytical, interpersonal, leadership and professionalism skills and abilities.
- Must have basic computer and math skills.
- Must have a valid North Carolina’s Driver’s License.
- Must submit to and pass pre-employment drug screening, criminal, HCPR and driving checks.

- Employee must be able to operate a vehicle for job duties, and keep a valid NC drivers license with insurance.
- The work environment will contain slight to moderate office-related noises. The employee is not exposed to extreme weather conditions, toxic fumes, or airborne particles.
- The employee must occasionally travel to different locations in the course of work.
