We are looking for a dedicated ACO Medical Director to join our expanding team in Southern California. This pivotal role involves leading value-based care initiatives that improve patient outcomes and reduce unnecessary healthcare utilization. You will work closely with providers and care teams to ensure high-quality care for frail, elderly patients in a dynamic, growth-focused environment.

Who We Are
We are a leader in delivering high-touch, compassionate care to high-needs patients. Our innovative approach uses advanced data analytics and patient-centered care strategies to reduce hospitalizations and keep patients in their preferred living environments.

Who You Are

  • A licensed physician in California, board-certified in Family Medicine, Internal Medicine, or Geriatrics.
  • A leader with a minimum of 3 years in medical group management or leadership.
  • Experienced in value-based care and passionate about transforming healthcare delivery.
  • Recently involved in direct patient care (within the last 2 years).

What You'll Do

  • Collaborate with an interdisciplinary team to develop and implement clinical standards.
  • Drive quality improvement and cost reduction initiatives.
  • Work with providers to enhance patient outcomes and manage healthcare costs.
  • Engage with patients and families to align care with patient goals.
  • Lead strategic efforts for market growth and ACO population expansion.

Location Requirements
This role requires being drivable to Costa Mesa and attending the office at least once a week.

Compensation and Benefits

  • $300K base salary with potential for performance bonuses, stock options, and a sign-on bonus.
