Job Summary:

As a Case Manager, you will play a crucial role in ensuring the well-being and quality of life for individuals with developmental disabilities. You will oversee various aspects of the Individual Support Plan (ISP), serve as a contact person, and coordinate essential paperwork and documentation. Your responsibilities also encompass conducting training, monitoring program compliance, and maintaining open communication with consumers, families, and colleagues.

Essential Job Duties and Responsibilities:

  • Assess the needs of individuals and identify any required services, such as equipment, clothing, or other essentials.
  • Attend and actively participate in Meetings to assess the level of support needed.
  • Complete bi-monthly DDP Notes to ensure accurate documentation of individual progress.
  • Conduct Nursing Assessments to evaluate and address the health and medical needs of individuals.
  • Develop community resources and connections for individuals on your caseload, promoting person-centered activities within their homes.
  • Establish and maintain monthly contact with family members to keep them informed and engaged in the care and support of their loved ones.
  • Participate in and complete all ISP (Individual Support Plan) Meetings, ensuring individual goals and objectives are met.
  • Review ISP goal tracking for the presence of documentation and the thoroughness of notes.
  • Visit homes as noted in Therap Case Note and document the date and time of each visit.

Requirements:

  • Bachelor’s degree in human services, psychology, sociology, or related field.
  • Knowledge of developmental disabilities services and regulations.
  • Must be a qualified individual DDP employed by or under contract with the agency.
  • Strong communication and interpersonal skills.

  • Must be able to communicate and converse with others, detect, and discern others’ communication, and express oneself.
  • Must be able to continually operate a computer and other office machinery such as a calculator, copier machine, and computer printer.
  • Must be able to lift and/or move up to 50 lbs.
  • Must be able to occasionally move about to access file cabinets, office machinery, and other locations in the building.
  • Must be able to operate a motor vehicle.
  • Must be able to relay information to all levels of staff.
  • Must be able to remain in a stationary position 50% of the time.
