The Behavioral Health counselor is a professional with specialized skills in the psychological, emotional, and social aspects of geriatric client care, with a working knowledge of an interdisciplinary medical model. This position displays sensitivity towards care recipients with chronic illnesses, mental health issues, and treatable illnesses in the elderly population. The counselor should be knowledgeable about grandchildren who have been traumatized due to the displacement of a family member and being raised by their grandparents.

Care Management Duties:

· Manages a Care Management caseload of clients and caregivers needing therapeutic counseling.

· Completes comprehensive assessments per Department of Elderly Affairs (DOEA) guidelines and on time per the admissions and readmissions process.

· Completes treatment plans with clients and families to assist with the interaction of clients.

· Assesses the social, cultural, and emotional factors of clients/caregivers/family.

· Coordinates individual/family counseling and group support to help the client/family cope with the care recipient’s illness.

· Educates grandparents on community resources and issues present in preschool or school environments.

· Complies with grant-related activities and mandates, including HIPAA and Morse Life Health System Policies.

· Identifies and intervenes with suicidal participants; assesses for abuse, neglect, or substance abuse.

· Identifies environmental safety hazards and risk factors related to emotional or suicidal individuals and educates the client/family accordingly.

· Completes all required documentation weekly to comply with clinical documentation.

· Attends staff meetings and all community outreach-related activities.

· Carries out duties and responsibilities as designated by the Clinical Director.

Palm Beach PACE Duties:

· Manages a caseload of PACE participants needing therapeutic counseling as determined by the PACE interdisciplinary team.

· Completes all comprehensive initial assessments on a timely basis upon receipt of approved orders from the interdisciplinary medical team.

· Completes all required EMR documentation weekly to comply with PACE clinical documentation standards.

· Coordinates implementation of behavioral health services to participant's homes, facilities, or PACE centers.

· Identifies and intervenes with suicidal participants; assesses for abuse, neglect, or substance abuse.

· Assesses social and emotional factors of participants based on assessment including issues related to diversity.

· Facilitates bi-weekly (2x per month) support groups at each PACE location to foster coping, improve mental health, and reduce loneliness.

· Attends and contributes to all appropriate IDT meetings including providing social, emotional, and mental health education to team members as appropriate.

· Maintains compliance with all HIPAA, Morse Life Health System, and PACE policy and procedural standards.

· Carries out duties and responsibilities as designated by PACE Behavioral Health Director.

· All other duties as assigned.

SUPERVISORY RESPONSIBILITIES

This position has no supervisory responsibilities.

QUALIFICATIONS EDUCATION and/or EXPERIENCE

To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

· Master’s degree in social work.

· Florida Registered Clinical Social Work Intern (RCSWI).
