Hospice Licensed Practical Nurse (LPN)

 in Mandeville, Covington, Slidell, and Hammond, Louisiana

We are currently seeking a dedicated and skilled Hospice Licensed Practical Nurse (LPN) to join our team. As a Hospice LPN, you will provide high-quality, patient-centered care. You will work closely with a multidisciplinary team to ensure comfort, manage symptoms, and offer emotional support to patients and their families.

As a Hospice Health Licensed Practical Nurse, here’s a big-picture view of what you’ll do:

  • Provide in house care to patients with six months or less to live, focused on quality of life
  • Work as part of an interdisciplinary team comprised of a physician, RN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
  • Perform nursing services which are planned, delegated, and supervised by the registered nurse
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
  • Take a holistic approach to care that engages the patient’s mind, body, spirit and family

A few must-haves for Hospice Licensed Practical Nurses:

  • Graduate of an accredited practical nursing program and active LPN license in Louisiana
  •  Hospice LPN experience required 
  • CPR certification 
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing, strong communication skills and compassion
