Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health Care Professional. As a vital member of our care team, you will work closely with patients and their families to provide high-quality, patient-centered care in the comfort of their own homes.

Key Responsibilities
  • Deliver skilled care to patients in their homes, working collaboratively with the Registered Nurse (RN) to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
  • Provide education and support to patients and their families, empowering them to take an active role in their care and recovery.
  • Document all care delivery and patient interactions in our electronic medical record (EMR) system, ensuring accurate and timely documentation.
  • Collaborate with the care team to ensure seamless communication and coordination of patient care.
Requirements
  • Current LPN license in good standing, valid state driver's license, and auto liability insurance.
  • At least one year of experience as a Licensed Practical Nurse in a clinical care setting, with a preference for home health experience.
