Hospice Registered Nurse Case Manager (RN)

As a Hospice Registered Nurse Case Manager, you will play a crucial role in providing compassionate care to patients facing life-limiting illnesses. This position involves delivering exceptional end-of-life support to patients and their families, ensuring comfort and dignity during challenging times.

Key Responsibilities:

  • Deliver home-based care focused on enhancing the quality of life for patients with limited life expectancy.
  • Collaborate with an interdisciplinary team, including physicians, LPNs, CNAs, HHAs, social workers, chaplains, and volunteers, to implement individualized care plans.
  • Conduct thorough patient assessments, monitor vital signs, evaluate respiratory function, manage pain, and administer prescribed medications and treatments.
  • Ensure the availability of necessary medical equipment, prescribed oxygen, medications, and other essential supplies.
  • Educate families about the patient's condition, updates to the care plan, medications, and hospice objectives.
  • Adopt a holistic approach to care that addresses the physical, emotional, and spiritual needs of patients and their families.

Qualifications:

  • Graduation from an accredited nursing program and possession of an active RN license in Louisiana.
  • A minimum of one year of experience in hospice nursing.
  • Current CPR certification.
  • Valid Louisiana driver's license, auto insurance, and reliable transportation.
  • Familiarity with state and federal home health regulations.
  • Strong clinical skills in nursing, excellent communication abilities, and a compassionate demeanor.
