GENERAL JOB SUMMARY

This is an administrative, management position. The Clinical Services Director will assist in creating the organization’s strategic plan, and will create a clinical department tactical plan that contributes to achieving the goals of the overall strategic plan. This position will oversee the clinical department and programs of our Hospice and Palliative services. They will work with the Executive Director, Human Resources Manager, MSS Manager, Operations Manager, Community Outreach Director and Finance Director to ensure ongoing communication is a priority between the clinical and other departments while monitoring timeliness of deadlines between departments, and being a champion when working with other healthcare providers. They are an independent thinker, problem solver, thinks “outside the box” and is a proficient communicator both with verbal and written skills who respects policies and ethical business practices. The Clinical Services Director will promote maximum performance while offering alternative solutions during busy periods due to our census and/or internal activities. This position is directly responsible for ensuring successful day-to-day clinical services delivery across all sites. Clinical services are provided under the direction of the Hospice Medical Director and/or Hospice Physician in accordance with the patient’s plan of care. The Clinical Services Director will be primarily responsible for overseeing the operational efficiency and quality of clinical services. The Clinical Services Director will lead efforts to improve quality outcomes and clinical services that contributes to an enhanced experience for staff and patients. This position carries supervisory responsibilities.

ESSENTIAL FUNCTIONS

Ensure that Hospice philosophy of care as defined by state and federal regulations are appropriately implemented.

Provide care consistent with the National Hospice and Palliative Care Organization standards of practice.

Keep the Executive Director fully informed.

Assists the HR Manager and clinical supervisors with recruiting, hiring, orientation and new staff for our Hospice and Palliative care programs.

Plans and directs Team meetings with clinical supervisors.

Works closely with and backs up the Compliance Nurse Manager in the creation and revision of HOH and MCSHC’s policies and procedures.

Oversees Polidori House’s six patient unit with 24-hour, day-to-day operations accountability.

In-depth knowledge of Federal, State and other CMS regulatory requirements.

Functions as the liaison with State surveyors, community health care partners and members of our communities we serve.

Identifies and organizes organizational opportunities for staffs’ continuing education, certifications, and licensures.

Coordinates with clinical supervisors in providing staff education regarding conditions of participations for Hospice and Palliative Care consults with essential staff when regulations are updated.

Makes supervisory field visits as necessary based on needs for the clinicians personnel competencies (to sign off orientations for new hires, check off staff annual competencies, or in situations regarding additional training for various competencies).

Coordinates with the clinical supervisors the annual performance reviews of staff. Supports clinical supervisors in preparing Performance Improvement Plans for staff when necessary.

Works collaboratively with other Agency Directors and Managers in meeting goals with specific timelines.

Oversees organizational activities including monitoring, trending and aggregation of specific outcome measures, ex. Quality Assurance and Performance Improvement (QAPI), quarterly/annual reports, quality measures, CMS require reports. Identifies key performance indicators.

Serves on committees internally and externally as requested by the Executive Director.

Attend and actively participate in senior staff departmental meetings as required.

May be Acting Administrator during the unavailability of the Executive Director.

Assure adherence to agency policies and procedures by clinical staff.

Ensures that all physicians’ orders are current and being followed by nursing personnel.

Plan, organize, coordinate, develop, implement, and manage Hospice of Havasu’s patient clinical services.

Prepare and present annual job performance evaluations for clinical supervisory staff in a timely manner.

Develop, administer and evaluate goals for the clinical department.

Supervise all nursing and therapy services provided by the organization.

Identify problems, recommend and implement solutions and evaluate outcomes.

Investigate incidents or unusual happenings, file reports and inform the appropriate individuals or authorities as required by policy and law.

Receive, investigate and make recommendations regarding patient complaints in accordance to hospice policy.

Participate in employee job analysis to eliminate hazards and prevent accidents.

Assist with investigating employee injuries.

Participate and provide leadership during IDT meetings and other meetings related to clinical practice.

Direct the orientation and supervision of new employees and students seeking experience in hospice care.

Hold regularly scheduled staff meetings at the Lake Havasu City and Ft. Mohave offices, identifying concerns; ensuing discussions are meaningful and valuable.

Provide documentation from the patients’ initial assessment and incorporate it into the plan of care and complete progress notes as required by hospice guidelines in a timely manner.

Provide input into computerized systems and monitor data related to patient care.

Participate in community educational opportunities regarding end-of-life care.

Promote hospice referrals in the communities of Lake Havasu and Ft. Mohave.

Develop and manage annual budget for the clinical department.

Ensure that care is provided in a respectful and dignified manner.

Practice safety in the workplace.

Maintain patient confidentiality at all times. Will discuss only those aspects necessary to the patient’s care and treatment with those directly involved in the patient’s care.

Ensure proper maintenance of medical records in compliance with local, state and federal laws.

May be required to do intermittent patient visits following the RN job description when

applicable.

Advocate for the patient’s best interest.

Fluent in the English language.

Ability to position self to move about, ascend, descend or traverse, transport, operate, activate, use, prepare, inspect or detect, perceive, identify, recognize, observe, inspect and assess are required. Ability to convey, communicate, speak clearly and exchange information, converse with, discern, and express oneself.

JOB REQUIREMENTS AND QUALIFICATIONS

Education:

Graduate of an accredited school of nursing.

Bachelor of Science in Nursing (BSN) with extensive hospice administrative experience.

CHPN Certification preferred.

MSN preferred.

Training Requirements: (Credentials, licenses or certificates)

Registered Nurse with current licensure in the state of AZ

CHPN Certification or must be able to obtain within one year after hire

Current CPR

Experience:

Five years of progressive administrative management experience including knowledge of health care policies and responsibilities.

Knowledge Requirements:

Demonstrated knowledge in hospice philosophy, medical aspects of hospice care including recent research, trends and pain and symptom management.

Comprehensive knowledge of nursing practice.

Business and management principles involved in strategic planning, resource allocation and leadership techniques.

Information and practices needed to treat human diseases including treatment alternatives, palliative care, drug properties and interactions.

Knowledge and support of agency’s safety and OSHA policies.

Understanding of HIPAA Privacy Standards and related agency policies.

Demonstrated understanding and compliance with agency policies, including but not limited to documentation, continuing education, work schedules and required HR documents, State and Federal policies and procedures, laws and regulations.

Stages of grief, death and dying.

Patient’s Rights.

Advance Directives.

Customer service principles.

Skills and Ability:

Successful experience as a clinical director or other health care management position.

Experience in clinical supervision and management of health care programs.

Is able to understand program budgeting, eloquently “paint the picture” in performance evaluations, and have knowledge of organizing efficient meetings and organized trainings.

Is a superb communicator with excellent people skills who is able to handle difficult situations while honoring the mission of the HOH Team, the agency’s Standards of Behaviors and Corporate Compliance Program.

Proficient in using Electronic Medical Records (EMR) systems, various computer programs (Word, Excel, Google Docs, Publisher, PDF/Adobe, Teams) and various hardware systems (desktops, laptops and/or tablets).

Structure and content of English language including the meaning and spelling of words, alphabetization, rules of composition and grammar.

Calculate general math computations.

Demonstrated skills in administrative performance, including written and oral communication, supervision of staff, documentation requirements, service coordination and case management.

Moderate computer skills in Microsoft Office applications.

Analyze information and evaluate results to choose the best solution.

Utilize proper body mechanics and safe working techniques.

Special Requirements:

Ability to operate a motorized vehicle and is auto insurable with minimum coverage as follows: $100,000 bodily injury per person, $300,000 bodily injury per accident and $50,000 property damage and have reliable transportation for work in the community and surrounding areas.

Ability to travel with-in the agency’s service area of 1.5 hours distance to/from the home office. Other travel may include occasional overnight trips to attend conferences etc.

On-call hours may be assigned to include nights, holidays and weekends. Hours may be flexible.

Participate in and complete all continuing education activities as assigned in a timely manner.

Follow all State of Arizona laws for Mandatory Reporting.

Adhere to Hospice of Havasu’s Standards of Behavior.
