Cardiology Case Manager RN

We are seeking a highly skilled and compassionate Cardiology Case Manager RN to join our team at Kaiser Permanente. As a key member of our multidisciplinary team, you will work collaboratively with physicians and other healthcare professionals to provide high-quality, patient-centered care to our members.
Key Responsibilities- Evaluate and identify members' needs, developing individualized care plans to address their unique requirements.
- Collaborate with primary care physicians, specialists, and other disciplines to develop and implement case management plans.
- Monitor and evaluate the effectiveness of care plans, making adjustments as necessary to ensure optimal outcomes.
- Coordinate interdisciplinary care, including utilization management, transfer coordination, and discharge planning.
- Act as a clinical liaison with outside agencies, such as County CCS, non-plan facilities, and third-party providers.
- Provide patient education and advocacy, empowering members to take an active role in their care.
- Develop and implement individualized patient education plans, focusing on self-management and disease-specific education.
- Facilitate patients' return to normal daily activities, making referrals for outside services and continued care as needed.
- Consult with internal and external physicians, healthcare providers, and discharge planners regarding continued care and treatment.
- Participate in data collection and analysis of clinical outcomes and customer satisfaction standards.
- Minimum two years of clinical experience as an RN in an acute care or ambulatory care setting.
- Bachelor's degree or equivalent experience required.
- Current California RN license and BLS certification required.
- Demonstrated ability to utilize and apply principles, practices, techniques, and methods of utilization review/management, care coordination, transfer coordination, discharge planning, or case management.
- Working knowledge of regulatory requirements and accreditation standards (TJC, Medicare, Medi-Cal, etc.).
- Computer literacy skills required.
- Bachelor's degree in nursing or healthcare-related field preferred.
- Case Management Certification or certification in the area of specialty preferred.
- Heart Failure experience preferred.
This is an 8-hour shift, with varied start times between 6:00 AM and 10:00 AM. Travel to San Marcos and future Cardiology sites in San Diego may be required.
