We are looking for an experienced MDS Nurse - RN - Registered Nurse or LPN - Licensed Practical Nurse.
** $33.43 per hour to $41 per hour based on skills and experience. **
MDS Nurse - RN - Registered Nurse or LPN - Licensed Practical Nurse Job Qualifications:
- Current licensure in the State of Massachusetts as a Registered Nurse.
- Minimum of 1 year of MDS Nurse experience in a long-term care setting.
- AANAC certification (RAC-CT) or the ability to obtain AANAC certification within 6 months of hire.
- Current BLS certification.
MDS Nurse - RN - Registered Nurse or LPN - Licensed Practical Nurse Summary:
- Interact with providers, insurance organizations, coordinating healthcare facilities, families and residents to resolve issues and provide clear communications.
- Work to ensure safe transitions through care for new admitting, re-admitting and discharging residents and allow the resident maximum benefit of their Medicare, Medicaid or other insurance benefit.
- Monitor Point Click Care “Care Management” areas to ensure accuracy and completeness of resident assessments.
- Monitor ABILITY programs dashboards (InfectionWATCH, RiskWATCH & CareWATCH) and maintain policies and for a comprehensive Reimbursement Program.
- Case mix clinical condition tracking.
- Implement auditing mechanisms.
- Oversee and assist with appropriate corrective action of identified deficiencies while reviewing Policies and Procedures.
- Cooperate in surveys and inspections.
- Ensures 672/802 is updated in PCC on a regular basis within required timeframe.
- Review CASPER Reports for accuracy.
- Ensures documentation is retained per home policy.
- Assist in the retrieval of case mix scores.
